PartⅠ THE VALUE OF DIFFUSION WEIGHTED IMAGING IN DIAGNOSIS OF CHRONIC HEPATIC SCHISTOSOMIASISObjectiveTo investigate the value of the apparent diffusion coefficient(ADC)value,exponential apparent diffusion coefficient(eADC)valueof in chronic hepatic schistosomiasis with different b values.Materials and Methods35 subjects confirmed as chronic hepatic schistosomiasis by clinical and pathologic methods and 38 subjects which had no liver disease were enrolled in the study.All of these subjects were underwent by conventional MRI and then by DWI scan.T1WI using fast gradient echo sequences.The same phase(IP and OP)were performed in the dual-scan.A fast recovery fast spin-echo sequence was used in T2WI.The b values were 0,50,600s/mm2in DWI.Choose the areas with low blood vessels and bile duct to set the(region of Interest,ROI).MeasurementADC1,ADC2,eADC1,eADC2values of hepatic ROI.ResultsChronic schistosomiasis group of ADC values were lower than normal liver gr oup,and eADC value was higher,the two were statistically significant(P<0.01).T he AUC underthe ROC curve were eADC2,ADC1,ADC2,eADC1withareduction.TheAUCw ere0.836,0831,0.829,0.748,respectively.Thespecificityof these parameters were 0.789,0.829,0.829,0.632 and the sensitivity were0.800,0.816,0.763,0.829,respectively.DWI are a under the ROC curve was 0.940,the specificity was 0.642 and the sensitivity wa s 0.943.ConclusionCompared with the control group,the liver disease group ADCweredecreased,eADC value increased,so,theDWI has a certain value in the diagnosis of chronic schistosomiasis liver disease.PartⅡ THE APPLICATION VALUE OF PROTONMAGNETIC RESONSNCE SPECTROSCOPY IN CHRONIC HEPATIC SCHISTOSOMIASISObjective To investigate the application value of proton magnetic resonance spectroscopy(1H-MRS)in chronic hepatic schistosomiasis.Materials and Methods30 chronic hepatic schistosomiasis subjects and 38 no liver disease subjects,which have been confirmed by clinical and pathological diagnosis,have underwent routine MRI and1H-MRS test.T1 WI using fast gradient echo sequences.The same phase(IP and OP)were performed in the dual-scan.A fast recovery fast spin-echo sequence was used in T2 WI.A point resolved selective spectroscopy sequence was used during the1H-MRSsacn.Choose the areas with low blood vessels and bile duct to set the(region of Interest,ROI).The scan should not use water saturated technology.Both the peak height and peak area of metabolites(water,cho,lipid)were measured.Results The MRS image of the two groups were mainly displayed as H2 O peak(4.6 ppm),Cho peak(3.2ppm)and Lipid peak(1.3 ppm);the water and Lip peak and the area under the two peaks of the chronic hepatic schistosomiasis group were lower than the control group(p < 0.05);while the Cho peak and the area under Cho peak showed no significant difference of the two groups(p>0.05).The AUC under the ROC curve were the water peak,area under water peak,area under lipid peak and lipid peak with a reduction.The AUC were 0.805,0.761,0.729,0.668,respectively.The specificity of these parameters were0.600,0.733,0.670,0.633 and the sensitivity were 0.921,0.737,0.789,0.737,respectively.MRS area under the ROC curve was 0.861,the specificity was 0.684 and the sensitivity was 0.867.Conclusion The1H-MRS has a certain value in the chronic schistosomiasis liver disease,especially the peak height and peak area of water.
