The Alteration And Significance Of Serum Concentrations Of OPG Before And After PCI

Abstract/Summary:
Obejects: To investigate the value of predicting acute coronary syndrome(ACS) and estimating it's prognosis by checking the alteration of serum concentrations of osteoprotegerin(OPG) before and 12h after pecutaneous coronary intervention(PCI), then comparing them with the serum concentrations of high-sensitivity C-reactive protein(hs-CRP) and cardiac troponin T(cTnT); to estimate the likely myocardium injury by PCI to revascularize.Methods: A total of 50 coronary heart disease patients who were going to have PCI, and divided into ACS group (n=20) and SAP group (n=30), then compared with control group(n=20) who were normality in coronary arteriography(CAG). We checked the serum concentrations of OPG, hs-CRP and cTnT before and 12h after PCI by ELISA of all the groups. We recorded the total time, pressure and times of balloon-expansion and the number of tentings we used in PCI in detail, then compared the results of all the items above and explored their correlations. We also concerned on the occurrence of major adverse cardiac events(MACE) in these three groups within 6 months'visit.Results:1 The serum concentrations of OPG, hs-CRP and cTnT were higher than SAP group and control group in the ACS group obviously before PCI(P<0.01); the serum concentrations of OPG, hs-CRP were much higher in the SAP group than the control group(P<0.01), but there was no difference in cTnT.2 There was no difference between ACS group and SAP group respectively in the serum concentrations of OPG, hs-CRP and cTnT before and 1h after PCI apparently, but they ascended 12h later(P<0.05). No difference before and after PCI in control group. 3 The raising of OPG concentration was positively correlated with the total pressure, time and times of balloon-expansion in PCI (P=0.001,0.001,0.007,respectively).4 The serum level of OPG was positively correlated with the level of hs-CRP and cTnT(P<0.001).5 Visit result: During 6 months'visit, 5 patients suffered from MACE, including one myocardial infarction, 2 recurrence of angina pectoris, one PCI-again and one death. MACE took place in those patients whose serum concentrations of OPG and hs-CRP were very high.Conclusions:1 OPG may be an valuable predictor of ACS.2 The less total pressure, time and times of balloon-expansion in PCI, the milder myocardial injury by PCI.3 OPG may be risk factors for patients after PCI.
