Study On Mass Health Of Weifang

Objective:Based on typical analysis and evaluation of the theory and practice of mass health in Weifang,Shandong province,this study provided experiences for further development of public health in Weifang and other areas.The concrete goals were:1 to explore the basis of mass health theory,elaborate the connotation of mass health,construct the mass health system,analyze the mass health theory framework;2 to review the process of mass health development, analyze the mass health practice,summarize the experience of mass health,show the mass health practice framework;3 to analyze the related data,and evaluate the comprehensive effect of mass health at the views of feasibility,effect,satisfactory;4 to put forward the policy proposals according to the above analyzes.Materials and methods:This study was an important part of" mass health"for Shandong province soft sciences research plan(A200632-7).The material was mainly from the references reviewing,on-the-spot investigation and the related people interviewing.Weifang was selected as the research object,and used the method which was qualitative & quantitative analysis to discuss the theory framework,the practice framework and the comprehensive effect of Weifang mass health,which mainly included three aspects:descriptive analysis(mean value,rate,constitution and frequency),statistic analysis(chi-square test,u-test of Poisson distribution of normal approximation,analysis of variance) and the economic analysis(the Lorenz curve, Gini's coefficient,Elasticity coefficient).Resouits:1 Discussion on the theory framework of mass health.1.1 Theory basis:the directivity of public health,the purposiveness of medicine,the comprehensiveness health elemental factors.1.2 Conception:the mass health means "the benefited,popular,participative health course ".1.3 Connotation:The mass health included many aspects,which reflected a kind of idea,a kind of culture.1.4 System:The system was summarized as "one idea,two goals,three mechanisms, four purposes,five innovations,nine jobs ".2 Discussion on the mass health practice framework.From 2001,the mass health practice framework was constructed on the basis of the big hygienic pattern,and the intervention system including the intervention in the inheritances,the environment,the life behaviors and the health services to provide the popular health service.3 The evaluation on comprehensive effects.3.1 The evaluation on feasibility.It analyzed feasibility of the emergence and development of the mass health on the basis of fundamental conditions,supporting measures and current progress.3.2 The evaluation on implementation effect.3.2.1 Health status of inhabitants.Compared with the national average levels of the infant mortality rate(5.78‰), the pregnant woman mortality rate(12.36/100,000),the children mortality under age 5(8.86‰),the newborn birth-defect rate(4.9‰),the average life expectancy(75.18 years old),disability rate(50 per thousand population),the legal reported infectious diseases incidence rate(93.15/100,000) in 2005,those of Weifang were far lower (u-test of Poisson distribution of normal approximation,were of significant statistic significances).The changes and tendency of indicators before and after implementation of mass health during the year of 2000-2005:the infant mortality rate decrease by 4.27‰(x~2=112.47,P＜0.0001,which was of significant statistics significance);the regnant woman mortality rate decrease by 9.41/100,000(x~2=2.98,P=0.70＞0.05,which was not of statistic significance);the children mortality under age 5 decrease by 4.74‰ (x~2=110.79,P＜0.0001,which was of significant statistic significance);the newborn birth-defect rate decrease by 0.23%(P＜0.0001,which was of statistics significance); the average life expectancy,which increased by 2.38 years,rised steadily,disability rate decrease by 13.4 per thousand population,the legal reported infectious diseases incidence rate during those years showed an upward tendency(X~2=20.23.P=0.0011＜0.05,which was of statistic significance);The knowledge rate for health education and the health behavior rate were all over 75%.3.2.2 Health service statusAccessibility:The distribution equity coefficient of physician in 2005 was 0.10. which was better than that in 2000(G=0.135):the distribution equity coefficient of hospital beds in 2005 was 0.125.which was better than that in 2000(G=0.15);the time inhabitants cost to go to medical departments was reduced greatly.Availability:The average health expense in 2000 for one person was 7.13%and 6.69%of the yearly income in city and country respectively,and that in 2005 was 5.82%and 5.72%;the average medical insurance payment in 2005 for one person was 83.33%and 10.45%of the health expense in city and country respectively;the inhabitants could afford the medicine expense in 2005 easier than that in 2000. However.compared with city.the medical burden in country was heavy.Suppliability:The projects and ability provided for the inhabitants by Weifang health and the correlated departments were increased,the quality of those was enhanced,but the gap between city and country was still exist.3.2.3 The participation statusCompared with the year of 2000.the activeness and enthusiasm of inhibits" participation were greatly enhanced in 2005:the allocations of hygienic resources and major structure showed a reasonable tendency,but the gap between city and country was still exist;The cooperation of departments was strengthened.3.3 The evaluation on the satisfaction.Through the questionnaires and interviews,we discovered that the inhabitants were highly satisfied with mass health,among which the inhabitants" approvement to the improvement of mass health convenience was the best(4.56±0.79),the health workers' approvement to the improvement of work atmosphere was the best (4.7±0.89),the government' approvement to the public health improved by mass health was the best(4.1±0.50).Discussion and suggestion:1 The mass health was a good choice to resolve the current problems in public health development.2 The idea of mass health proposed three basic principles which included the direction of public health development,the mechanism of health work,and the criteria of health work instruction to promote the development of public health.3 The harmonious development perspective of public health.
