| ObjectiveTo sum up the cases of ovarian tumor in Shandong Provincial Hospital during the latest 10 years, and to investigate the histology types of ovarian tumors and their age distributions.MethodsBetween Jan 1st, 1998 and Jan 1st, 2008, 1537 cases of inpatients diagnosised as ovarian tumor in Shandong Provincial Hospital were collected with the aid of computed case system,and 1447 cases were selected as objectives according to the criteria.The ages and pathological diagnosises of paraffin section were recorded, and the cases were classified according to 2003 WHO histological classification of ovarian tumor. Microsoft Excel 2003 was applied to set the data and statistical analysis was done by SPSS 16.0.Results1. According to 2003 WHO histological classification of ovarian tumor, there are 7 types of 41 kinds in 1447 cases.Germ cell tumor is the most common type , which is amounted to 703(48.58%). Surface epithelial-stromal tumor is amounted to 590(40.77%), and sex cord-stromal tumor is amounted to 114(7.88%). Among the 1447 cases, there are 1100 benign tumors and 347 malignant ones(including borderline tumors). The ratio of benign, borderline and malignant is 25.58/7.01/1. The common benign tumors are benign germ cell tumor (662 cases ,60.18%), benign surface epithelial-stromal tumor (351 cases, 31.91%) and benign sex cord-stromal tumor (87 cases,7.91%).The common malignant tumor are malignant surface epithelial-stromal tumor (196 cases,56.48%), malignant germ cell tumor (41 cases,11.82%) and malignant sex cord-stromal cell tumor (27 cases,7.78%).2.There is a difference in proportion for different histologic type of ovarian tumors according to the age.Germ cell tumor is the major type among the patients less than 40, however, surface epithelial-stromal tumor showed signaficantly higher incidence among the people older than 40, and the propotion is increased with the age.3. There are 2 peak ages of malignant ovarian tumor, patients less than 20 and over 40. The incidence of malignant ovarian tumors sustained at a high level, but does not increase with the age among people over than 40. During 21 to 40 ,women have the highest incidence of ovarian tumor with most of which are benign tumor.4. Serous tumor is different from mucinous one in malignant proportion and age distribution. Serous tumor has higher malignant proportion than mucinous tumor,however,mucinous tumor has higher benign and borderline proportion than serous ones.The average age of malignant serous tumors is 53.03,which is significantly higher than benign ones(42.49).But the average age of malignant and benign mucinous tumors are close.Malignant serous tumor(53.03) has a higher average age of onset than mucinous tumor(45.05).Conclusions1 .There is a difference in constituent ratio of histological type of ovarian tumors at home and abroad.2. The incidence of malignant ovarian tumors are on the rise, the surface epithelial-stromal type of malignant tumor has an upward trend year after year.3. There are 2 peak ages of malignant ovarian tumors,patients less than 20 and over 40.We should pay more attentions to the patient with ovarian mass in peak ages.4. Serous tumor is different from mucinous one in malignant proportion and age distribution.Age is helpful in the diagnosis of malignant serous tumors.As mucinous tumors are concerned, careful examinations should be taken to avoid missed borderline tumors.5. Age is useful in the discrimination of benign and malignant tumors.6.Benign tumor (including tumor like conditions) has a rate of malignanttransformation,and we should pay attentions to it. 7. In order to avoid misdiagnosis, we should not ignore the rare type ovarian tumor. |
