Objective To assess the value of depiction of ovarian vein in the differential diagnosis of the uterine and ovarian origin solid tumor at MSCT. Methods The MSCT images of 71 cases with pathologically confirmed uterine and ovarian origin solid masses(total 99 masses) were retrospectively reviewed by blinded radiologists with conventional method (method 1:to analyze the location,relation to the surrounding tissue,organs,the shape,borders,attenuation and enhancement characteristics of the mass) and conventional method + determination the relationship between the ovarian vein and the mass (method 2). Results The sensitivity, specificity, misdiagnostic rate, false negative rate, Youden's index, positive likelihood ratio, negative likelihood ratio, accuracy ,positive predictive value and negative predictive value of method 1 and method 2 were 81.08%,64.00%,36.00%,18.92%,0.45,2.25,0.30,76.77%,86.96%,53.33%and 90.54%,92.00%,8.00%,9.46%,0.83,11.32,0.10,90.91%,97.10%,76.67%,respectively,there was statistically significant difference between two groups(P<0.05). The presence of the ovarian vascular pedicle sign was identified in a total of 75% of ovarian tumor. this is a reliable sign for confirming the ovarian origin of a pelvic tumor and for differentiating ovarian tumor from non-ovarian masses.This sign was not observed in uterine tumor. In contrast, in 72% of uterine masses, the ovarian vein tended to show abrupt interruption or continuing to the normal ovary. Conclusion Combined use of conventional method + determination the relationship of ovarian vein to the mass and vascular pedicle sign are of a higher sensitivity and specificity in differentiating the ovarian tumors from non-ovarian masses.
