Objective:To discuss the clinical feature of the fungal infection with Children ,itsrisk factor and the monitor of the drun tolerance .Methods:restrspective analysising six hundred and forty children,s clinical data with positive fungal culture,diving them into four groups ,accoding to liver transplantation ,ICU,blood,common.The infection feature and risk factors ,the feature of the drug tolerance were compared retrospectively according to their clinical data with statistics.Results: (1) Candida albicans (65%) was the common pathogenic fungus among the 640 fungal strains. Twelve (12/16,75%) recipients were diagnosed as C. albicans infection in the 22 case of liver transplantation. Candida parapsilosis infection had the trend of escalation (14.3%), was higher than other reports obviously.(2) Respiratory tract and digestive duct were the most common infectious sites . (3)The drug-resistant rate of C. albicans to 5-fluorocytosine amphotericin B and azoles was 1.9%,1.7% and >80%. C. parapsilosis had the most sensitive to all antibiotics.(4)In blood department and ICU, The combined use of antibiotic in blood department and ICU, which length of hospital stay≥7 days, age≤3 years of age are risk factors for fungal infections. The fungal infections RR value of the age of≤3-year-old is 11.1 times of the age of 11.1 years of age≥3-year-old. (5)The patients from blood department and ICU were divided into anti-fungal treatment group and non-anti-fungal treatment group. There's no statistic difference between the improvement rate and the cure rate from the two groups (p>0.05). So if the result of fugal culture was positive only once, we could not diagnose the patient as fungal infections. (6):the fungal infection of liver transplantation is focused on 1-4weeks after the surgons. The position of the infection is mainly in the respiratory tract and the alimertary tract ,and often the fungal infection is occurred with more than two kinds of the bacterial infection.(7):In the risk factor analysis of the fungal infection of the liver transplantation ,only the cotermportary P<0.05,the length of stay ,the using of cyclosporin,using of lmipinem,s P are all >0.05,may be because the sample capacity is just minor.(8):the Fluconazole and teaconazole should be used before live transplantation to plantation to prevent before hand the fungal infection .(9):the result of drug sensitive test of the fungal prompted that the fungal is resistance to the Fluconazole ,but using the Fluconazole is still effective on the clinic.Conclusion(1) Candida albicans is the mainly nosophyte of the children's fungal infection ,the posions are respiratory tract and alimertary tract.(2) age,using immunodepressant ,using antibiontics associationed are the independent risk factors of the fungal infection.(3) the Fluconazole and teaconazole should be used before live transplantation to plantation to prevent before hand the fungal infection .
