Objectives: To find out the historical features which have higher value in migraine diagnosis and differential diagnosis, to help clinical practitioners diagnose migraine more rapidly and accurately.Methods: 250 patients who have primary headache were chosen in this study (include 117 migraineurs). Each patient's history was taken in detail with a semi-structured questionnaire of headache history which developed by ourselves, then each patients received a clinical interview of neurology expert and the expert give him or her a diagnose about his or her headache using the IHS critera. Finally each historical feature's diagnostic value were analyzed by statistic methods.Results: In all the historical features that we studied: pain exacerbated by activity ,pain throbbing, disability , phonophobia, nausea ,photophobia, pain on oneside, both have high sensitivity and high specificity , the Youden index (95%CI) of them are: 0.59 (0.49~0.69), 0.57 (0.47~0.67), 0.50 (0.40~0.60), 0.46 (0.36~0.57), 0.41 (0.30~0.52), 0.40 (0.31~0.50), 0.34 (0.22~0.45).Conclusion: The study was taken in Chinese for the first time, and the conclusion of the study reflect the specialty of ourown. Known the sensitivity and specificity of each historical features, can help us to diagnose migraine rapidly and accurately.
