Background:In the 2010 Global Disease Survey,migraines ranked third in the disease epidemic spectrum,ranking seventh among disabling diseases.The WHO 2016 Global Burden of disease survey showed that migraines were the second highest among 328 chronic diseases in terms of the healthy life Year for disability loss(YLDs).There is a significant disparity in the prevalence of migraines between men and women,and many studies around the world have shown that the prevalence rate of migraine in middle-aged people is as low as 1:(3-4).The prevalence of migraine has a large difference in sex and age,suggesting that sex hormones may play an important role in the development of migraine.Previous studies have focused on estrogen or hypothalamic-pituitary-ovarian axis in the pathogenesis of migraine and estrogen replacement therapy in patients with menopausal migraine,with less reports of androgen research.Some studies have found that the good age of migraines is consistent with the period of decline in androgen levels,suggesting that there is a link between androgens and migraines.Clinical studies have shown that testosterone and its synthetic derivative therapy can prevent migraine attacks,both men and women have an effect.Objective:To study the changes of androgen level and its free index in migraine patients and the comparison of androgen levels in patients with different types of migraine.Methods:Forty five patients with migraine were selectedfrom November 2017 to January 2018 in the Outpatient Clinic of ShandongProvincialHospital,and 45 healthy people with sex and age matching(± 3 years)were selected as health control.Headache diagnosis is based on ICHD-??diagnostic criteria to eliminate other types of primary headaches such as tension-type headaches and secondary headaches,and to record in detail the basic information and clinical characteristics of all patients.All the subjects were in the laboratory of our hospital to complete the determination of TO,SHBG and%FTI.To observe the difference of androgen level between 2 groups,compare the difference of androgen level between migraine with aura(MwA)and migraine without aura(MWoA),and the family history of androgen level and migraine in migraine patients,headache course,frequency of attack,duration of attack,severity and related accompanying symptoms,The history of previous migraine treatment and other characteristics of correlation analysis.Results:(1)There was no significant difference between TO,SHBG and%FTI in male migraine patients and healthy control group(P>0.05),and TO(0.26±0.09)and%FTI(1.12±0.99)in female migraine patients were significantly lower than those in healthy control group(?2=10.554,P=0.004;?2=9.145,P=0.004).SHBG(78.5±28.2)was significantly higher than in the healthy control group(?2=6.489,P=0.035).(2)There was no significant difference in TO,SHBG and%FTI between male migraine with aura and migraine without aura(P>0.05),and there was no significant difference between women(P>0.05).(3)The levels of TO,SHBG,%FTI and headachecourse(r=0.095),headache frequency(r=-0.058),headache duration(r=-0.183),VAS scores(r=0.097)and MIDASscores(r=0.109)were not found significant correlation in male migraine patients(P>0.05).There was no significant correlation among women with migraine(P>0.05).The TO levels of patients with migraine were negatively correlated with the headachecourse(r=-0.892)and the headache frequency(r=-0.756)respectively(P<0.05),and had no significant correlationwith headache duration(r=-0.121),VAS scores(r=-0.098)and MIDAS scores(r=-0.054)respectively(P>0.05).Conclusion:(1)Compared with the healthy controls,the androgen levels in female migraineurs were significantly reduced,while the male migraine patients had no significant changes in androgen levels.(2)There were no significant differences in androgen levels between migraine with auraand without aura.(3)Androgen levels were negatively correlated with headache course and headache frequency in migraine patients,and had no significant correlation with headache duration,VAS scores and MIDAS scores.
