Clinical Study On The Relationship Between TCM Sydrome Types Of Breast Cancer And Adiponectin, Leptin, Vascular Endothelial Grouth Factor And Part Of Immunohistochemical Factors

Object:To explore the relation between TCM Sydrome types of Breast Cancer and adiponectin, leptin,vascular endothelial grouth factor and immunohistochemical Factors ER,PR,C-erbB2,Ki67,et al.In order to provide objective evidence for TCM syndrome differentiation of BC and to conduct the clinical treatment.Materials and methods:Seventy-four outpatients and inpatients were chosen according to the selecting standard.All of them were suffered from Breast Cancer and treated on the traditional Chinese medicine department and surgery department of Huashan Hospital,during the period from Dec.2008 to Mar.2009.Before the operation,a clinical assessment and differentiation of symptoms and signs for classification of syndrome on TCM must be made.All patients could be divided into YIN -deficiency syndrome group,or liver stagnation syndrome group,or Phlegmdampness Due to Deficiency of the Spleen Syndrome group,or Disharmony of thoroughfare and Ren channels syndrome group,or Qi deficiency syndrome group.Parts of Immunohistochemical Factors as ER,PR,cerb-B2, Ki67 and serum adiponectin,leptin,vascular endothelial grouth factor,blood sugar (BG),Insulin(INS),Insulin Sensitivity Index(ISI),blood fat all were be measured after the operation.Analyse data by statistics software.Results:①The level of serum leptin and INS2h in the group of phlegm-dampness due to deficiency of the spleen syndrome is higher than other groups remarkably(P＜0.01),but serum adiponectin is the lowest among the five groups,there was an obvious significant between them(P＜0.05)②Remarkably higher serum VEGF was observed in the group of liver stagna- tion and Disharmony of thoroughfare and Ren channel(P＜0.01).③Positive rate of the expression of ER,PR in the group of Qi deficiency syndrome is higher than other groups,and it revealed a significant statistic differ- rence of the expression of ER(P＜0.05).④There were no statistical differences of age,course of diseases,clinic stage, BMI,ISI,blood sugar,blood fat and the expression of PR,c-erbB2 among all groups.Conclusion:①Breast cancer patients in the group of Phlegm-dampness Due to Deficiency of the Spleen Syndrome is high risk of accompanying obesity,insulin resistance, hyperinsulinemia;hyperinsulinemia and the increasing adipose cytokine that is due to obesity can promote the occurrence development and metastasis of BC.These observations suggest that patients in this group have a poor prognosis.It is important to use drugs which can reinforce spleen to relieve dampness and remove phlegm.②The increased serum VEGF in the group of liver stagnation and Disharmony of thoroughfare and Ren channels suggest that it is powerful of its tumor angiogenesis in this two types,which also can suggest that it is easy to transfer.These findings indicative of a poor prognosis.The patients of this type must be use promoting blood circulation and removing blood stasis herbs.③The occurrence development of BC in the group of Qi deficiency syndrome may be related to estrogen,progestogen,patients in this group may promise a good prognosis.④There were no statistical differences of age,course of diseases,clinic stage, BMI,ISI,blood sugar,blood fat and the expression of PR,c-erbB2 among all groups.It might be related with the small number of cases.
