OBJECTIVEThe aim of the present study was to detect the periodontal pathogens in the subgingival plaque of periodontitis patients with type 2 diabetes,analysis the relationship between periodontal pathogens and systemic factors and periodontal status in type 2 diabetic patients,discuss the role of the periodontal pathogens in the chronic periodontitis and type 2 diabetes mellitus.METHODS1.Study subjectsThere were 154 subjects with type 2 diabetes mellitus,as comparison,we selected 120 subjects with periodontitis,40 healthy subjects whose age,gender were consistent with type 2 diabetes mellitus subjects.Subjects were excluded from the study if any of the following conditions applied,other systemic diseases,professional tooth cleaning or periodontal therapy and took any antibiotics and contraceptive agent during 3 months preceding to the study,women with gestation.All subjects enrolled into the study signed the informed consent form.2.Measurement of clinical periodontal conditionWe selected 16,11,26,36,41,46 as index teeth.Clinical periodontal parameters included probing depth(PD),clinical attachment level(CAL),sulcus bleeding index (SBI),simplified oral hygiene index(OHI-S) and mobility(M) were measured and recorded by examiner.3.Samples collectionSubgingival plaque samples were obtained with sterile subgingival curettes in the buccal site of the tooth 16,11,26,36,41,46.4.Detection of pathogensPathogens genomic DNA were extracted by phenol and chloroform from plaque,A 16S rRNA- based polymerase chain reaction(PCR) detection method was used to determine the prevalence of Pg,Aa,Fn,Pi,Tf,Td in the subgingival plaque samples. The AVG of positive band in the agarose gel electrophoresis was analyzed using the image analyzer software.The relative quantites of Pg,Aa,Fn,Pi,Tf,Td were determined.5.Statistical analysisThe data were statistically analyzed using SPSS11.5 software.Chi-square test, independent-samples and paired samples t test,pearson correlation and logistic analysis were used in this study.Significance level:P<0.05 is considered statistically significant, P<0.01 is considered highly statistically significant.RESULTSThe detective ratio of Pg,Aa,Tf in DM group was significant higher than it in the CP group,and the relative quantity of Aa,Tf in DM group was significant higher than in CP group,while,the relative quantity of Pi in DM group was significant lower than in the CP group,respectively(P<0.05).In DM group,there was no relationship between the detective ratio of Pg,Aa,Fn,Pi,Tf,Td and the value of FBG,PPBS,TC and TG.There was positive correlation between the relative quantity of Tf and the value of FBG,while,there was negative correlation between the relative quantity of Pi and the value of FBG and TC. CONCLUSIONSWhen the periodontal status was consistent,the pathogens in the subgingival plaque were different in the periodontitis patients with and without type 2 diabetes mellitus.Pg,AaZ Tf were dominant pathogens in the periodontitis patients with type 2 diabetes.Except Tf,Pi,other periodontal pathogens like as Pg,Aa,Fn,Td have no relationship with the level of blood glucose.
