The Clinical Observation Of Treating Un-cracked Oviduct Pregnancy Conservatively With Three Methods

ObjectiveTo explore the therapeutic effect and the mechanism of ectopic pregnancy(EP) conservative treated with yunnanbaiyao capsule and fukeqianjin capsule combined with western Medicine. Ectopic pregnancy(ectopic pregnancy,EP) is the fertilized egg in the uterus outside the body cavity implantation.learning referred to as the "pregnancy" is a common acute abdomen in obstetrics and gynecology,and that is also early during the primary cause of death for pregnant women.Over the past decade the incidence of ectopic pregnancy has shown a rapid increase in the trend of the gestational age have shown a serious threat to the safety of the women.However, as the patients with self-protection awareness,as well as the extensive use of ultra-B,high sensitivity detectivation of bloodβ-HCG and the common use of diagnostic laparoscopy,80%of the ectopic pregnancy can not be diagnosed before rupture.Early diagnosis of ectopic pregnancy create the conditions of conservative treatment.And early diagnosis requests more and more which have many of the patients requested the retention of the fallopian tube,therefore,conservative treatment is getting more and more attention.Application of methotrexate author and gynecological United Yunnanbaiyao capsule and Fukeqianjin capsule in the treatment of ectopic pregnancy to explore the clinical efficacy and mechanism of the traditional Chinese and western medicine to the conservative treatment of unruptured ectopic pregnancy.Methods105 cases of ectopic pregnancy were randomly divided into 3 groups.A group of 35 cases of use the proprietary Chinese medicines Yunnanbaiyao Capsule capsule and Fuke Qianjin capsule united mifepristone(RU486) with highly dose treatment,beginning on the 1st day of oral mifepristone,a dose of 100mg/time,2 times/day, for 7 consecutive days,a total of 1400mg,the same day to start taking Yunnanbaiyao and Fuke Qianjin capsule,the dose of Yunnanbaiyao 3 capsules/time,2 times/day,Fuke Qianjin 4 capsules/day,2 times/day,taking the turn of the two drugs,one course of treatment is that Yunnanbaiyao 2 capsules,4 capsules one Fuke Qianjin in the treatment.(30 capsules Yunnanbaiyao/bottle, Fuke Qianjin 24 capsules/box).B group with 37 cases of proprietary Chinese medicines Yunnanbaiyao capsule and Fuke Qianjin capsule united with methotrexate(MTX) integrated in the treatment.MTX hip deep muscle injections,each dose 20 mg on every day for the medication,a total of 5 times(1,2,3,4,5,in the first therapy course).Beginning on the 1st day of oral capsules of Fuke Qianjin and Yunnanbaiyao capsule,which use two times a day both.33 cases of Group C united mifepristone with methotrexate in the treatment, the use of mifepristone is that a dose of 50mg/time,2 times a day for 3 consecutive days a total of 300mg.The use of methotrexate is the same with B group.Three groups were in the first eight days to re-check the treatment of bloodβ-HCG,blood,liver and kidney functions,if the decline of the bloodβ-HCG is below 15%,we will carry on a second course of the treatment,the use of the three groups of drugs are same with before.B,C two groups can be a one-time intramuscular injection of MTX with 50mg per square meter of body surface area.Before the second course of the treatment we shoud re-check the first eight days of bloodβ-HCG,blood,liver and kidney function.The time between the two course of the treatments is for 5-7 days.To observe the clinical efficacy of each group,the decline of the bloodβ-HCG,reduce the mass of ectopic pregnancy,when the pain disappeared and the situation of how the drugs are used.ResultsRegarding to the total cure rate,the B and C group is higher than the A group(P<0.05).But the cure rate of the B and C group in the first course of treatment is higher than that of the A group(P<0.05) and the difference of the cure rate of the three groups in the second course is insignificant(P>0.05),so the significant difference of the cure rate actually happens in the first course of treatment.The serumβ-HCG normalizing time and the abdominal pain duration time of the B and C groups are shorter than that of the A group(P<0.05).Regarding to the reduse speed of EP mass,the B group is obviousily quicker than the other groups(P<0.05).The total occurrence rate of the medicine side reaction of the A and B group is obviously smaller that of the C group(P<0.05).ConclusionThe conservative method with yunnanbaiyao capsule,fukeqianjin capsule and Western medicine to treat EP is one of the best method,because of the high efficacy and small side effects.It will help to maximize the recovery of the reproductive function to the patients and to keep them in good health.
