Analysis Of Surgical Therapy For Ulcerative Colitis: A Report Of 22 Cases

BackgroundUlcerative colitis(UC) belongs to inflammatory bowel disease(IBD), the etiology of UC remains unknown,now it is considered as a chronic non-specificity inflammatory disease of rectum and colon. The lesion located in mucosa and submucosa of rectum and colon. Clinical manifestations include diarrhea, bloody stools and stomachache. The main therapy of UC is medical treatment, including 5-aminosalicylates (5-ASAs), glucocorticoids, antimetabolites and antibiotics. China reported that 20%-30% UC patients must undergo surgical therapy because of failure of medical therapy. On the condition of correct opportunity and proper operation type, surgical therapy can improve patients'life quality.Methods22 cases of UC with surgical therapy were gathered,which were admitted during 2002.7 and 2007.12 for a retrospective study. The clinical data including original data, clinical manifestation, medical treatment, surgery indication, operation type and short-term outcome and long-term outcome.ResultsIn this study,surgery reason of 11 cases is failure of medical treatment,surgery reason of 7 cases is complication ( enteric cavity stenosis,, intestinal obstruction, perforation of intestine and toxic megacolon ), surgery reason of 2 cases is severe UC, surgery reason of 1 case is the side effect of steroid, surgery reason of 1 case is polyp intestinal. 2 cases underwent partial colectomy,13 cases with total proctocolectomy and ileostomy ,3 cases with total proctocolectomy and ileal pouch-anal anastomosis(IPAA), 3 cases with IPAA and prophylactic ileostomy, 1 case with Hartmann. All the followed-up 16 cases UC patients' life quality is improved.ConclusionOn the condition of correct opportunity and proper operation type, the short-term and long-term outcome of surgical therapy for Ulcerative colitis is satisfaction.
