Acute respiratory distress syndrome(ARDS) is characterized by progressive dyspnea and refractory hypoxemia following a variety of direct and indirect insults to the parenchyma or vasculature of the lungs. These insults include severe infection, trauma, shock etc.Acute respiratory distress syndrome is the serious stage or type of acute lung injury(ALI).With the improvement of the modern recovery technology and the level of rescuing critical patients, the innovation of the treatment method and drug, especially the progress of the infection control, enteral nutrition, low tidal volume ventilation strategy which can protect lung, mortality from early lung injury decreased. But mortality from ARDS is substantial, still remained at 40%-60%,and current therapy primarily emphasizes mechanical ventilation and judicial fluid management plus standard treatment of the initiating insult and any known underlying disease. The authors of this article present five patients with ARDS since the hospital has been opened.By the comparison on diagnosis and treatment on these five cases, combined with the literature on ALI/ARDS,the causes of ALI/ARDS,diagnosis and treatment are discussed in this article.
