| Objective:Multiple myeloma is a vicious disease with plasmacyte proliferating. Chemotherpy remains the primary treatment of MM today. The regiment of VAD becomes a research hotspot because of it's good therapeutic effect.To compares the therapeutic effect between MP ( M2 ) regiment and VAD regiment in the newly-diagnosed MM patients.Methods:41 newly-diagnosed cases with MM which were diagnosed between June 2000 and January 2009 were collected from Quanzhou first hospital. The diagnostic criteria is based on internal diagnostic criteria,and they were treated with MP(M2)regiment and VAD regiment respectively.Blood cell analysis ,M protein ,blood calcium,renal and Liver function,bone marrow aspiration and bone imaging were chosen as evaluation indexes.Results:1. Effective rate :There were 18 cases treated with VAD regiment: the totally effective rate is 88.9%,partial remission 9(50%),improvement 7(38.9%),inefficacy 2(11.1%). There were 23 cases treated with MP(M2)regiments :the totally effective rate is 56.5%,partial remission 6(26.1%),improvement 7(30.4%),inefficacy 10(43.5%). The statistics analysis shows P=0.024(P<0.05),which means the VAD regiment is better than MP(M2)regiments in therapeutic effection. The recovery rate of functional lesio to kindeys in VAD regiment is 100%, which is 50% in MP(M2)regiments. The statistics analysis shows P=0.04(P<0.05),which means the VAD regiment injurys the kindeys less than MP(M2)regiments. The recalcification rate of osteolytic lesio is 77.8% in VAD regiment,and 34.8% in MP( M2)regiments. The statistic analysis shows P=0.006(P<0.05),which means the recalcification rate of osteolytic lesio in VAD reigment lower than that in MP(M2)regiment.2.Myelosuppression: Myelosuppression rate was 89.9% in VAD regiment ,which was 69.6% in MP(M2)regiments. There were no significant differences of myelosuppression between the two groups, because the statistic analysis shows P=0.138(P>0.05). But when we compare the myelosuppression rate among the three groups of VAD,MP and M2, we found that the severest level of the myelosuppression is MP regiment, and the VAD regiment has an advantage in the severity of myelosuppressioni(P<0.05).3.Other side effects:There were no significant diffrences (P>0.05)between VAD regiment and MP(M2) regiment in the incidence rate of infection,digestive system side effects, peripheral nervous system injury and hepatotoxicity after chemotherapy.Conclusion:1. The VAD regiments have notably more advantage in remission rate on the newly-diagnosed MM patients than MP(M2)regiments.Therefor, it should be considered as the preferred regiment.2. VAD regiments have a lower myelosuppressioni severity level than M2 regiments,and other side effects are also gentle during the treatment. |
