Objecttive:The cerebrovascular disease is the commonly encountered and multiple disease of middle and old age.It is one of the three greatest causes of deaths of humanity.It has four big characteristics:high occurrence rate,high reoccurrence rate,high cripple rate and high mortality rate.It is the big enemy of human health.The cerebrovascular disease taking place in posterior circulation is more harmful to human health.PCI is the commonly encountered ischemic cerebrovascular disease,it shares 20%of ischemic cerebrovascular disease,it's mortality rate is up to 20%~30%of it's total.Therefore,to understand it correctly and master the.essential examination methods of it is very important.Thus,we can reduce it's mortality rate and cripple rate by confirming diagnosis and curing it immediately.The purpose of this study is to inquire into the diagnosis value of MRI and BAEP in PCI so as to provide diversified diagnosis methods to it.And we can provide accurate diagnosis basis for the patients with PCI.Methods:In this study,Seventy-seven patients with PCI were collected from May 2007 to February 2009 in department of neurology of our hospital.All these patients were attacked for the very first time and the time of being attacked was from 4 hours to 3 days.All these patients were matched with the diagnosis standards of expert's consensus of PCI in 2006 and the fourth national cerebrovascular disease meeting in 1995.These patients included 30 cases with posterior circulation TIA and 47 cases with posterior circulation infarction.①The posterior circulation TIA group(30 cases) included 18 men and 12 women,with a mean age of 66.24 years(range 42-83 years,SD 7.32);②The posterior circulation infarction group(47 cases) included 27 men and 20 women,with a mean age of 67.33 years(range 40-85 years,SD 6.52).③In addition,40 normal men were also collected into control group during the same peried to collect normal reference value of BAEP.The control group included 24 men and 16 women,with a mean age of 65.45 years(range 42-80 years,SD 7.54).All patients were examined with Somtom Volume Zoom 4 line spiral CT equipments made in Siemens company so as to expel intracerebral hemorrhage and other disease in posterior circulation during 24 hours.All patients were examined with Time-avantol 1.5T superconductive MRI equipments made in Siemens company and KEYPOINT? 4 evoked potential instrument made in Medtronic company to acquire original data during 24 hours.The control goup also received the same examinations,and patients were abnormal examining with CT and MRI did not be collected into this group.The statistics were made by SPSS 13.0,and the difference between measurement data were decided by analysis of variance,the difference between the numeration data were assessed by X2 test.Result:1.The result of BAEP in control group and the comparison with posterior circulation TIA group and posterior circulation infarction group:All waves of BAEP in control group could be evoked and the shape of all waves were good。Part of waves of some cases in two test group could not be evoked or the shape of some waves were not good.Comparing with control group,the PI and IPL of all vaves of BAEP that could be evoked in two test group had statistical difference(P<0.05).2.The result of posterior circulation TIA group:Among 30 cases in this group,4 cases were positive(13.33%)examined by MRI,14 cases were positive(46.67%) examined by BAEP,Two positivity rates of the two examination methods had statistical differrence.The positivity rate of BAEP was higher than MRI(P<0.05).3.The result of posterior circulation infarction group: Among 47 cases in this group,43 cases were positive(91.49%) examined by MRI and 25 cases were positive(53.19%) examined by BAEP,Among 25 cases of posterior circulation infarction being positive examined by BAEP,22 cases were of brainstem infarction,including 1 case of medulla oblongata infarction,1 case of midbrain infarction,20 cases of pons infarction,another 3 cases having no accurate infarction spot examined by MRI.Two positivity rates of the two examination methods had statistical difference.The positivity rate of MRI is higher than BAEP(P<0.0005).4.The comparison of the diagnosis values of two examination methods in posterior circulation TIA group and posterior circulation infarction group:The positivity rates between the two test group examined by BAEP had no statistical difference (P>0.05),but had statistical difference examined by MRI,the positivity rate of posterior circulation infarction group was higher than posterior circulation TIA group(P<0.0005).Conclusion:1.BAEP could provide some diagnosis basis to posterior circulation TIA and posterior circulation infarction.2.MRI had accurate diagnosis values and position values in posterior circulation infarction,and it s positivity rate was up to 91.49%,and was higher than BAEP.3.BAEP was more sensitive than MRI to the ischemic disease implicating auditory pathway and could diagnosed some cases of PCI implicating auditory pathway which MRI could not diagnosed.4.BAEP and MRI had complementary value in diagnosing PCI.
