Speciality:Pediatrics of Traditional Chinese MedicineAuthor:Zelin WengTutor:Professor Youjia XuChildren' s chronic cough usually takes cough as the sole or main symptom. Patients cough more than 4 weeks and their chest X-ray examinations have no significant abnormality.Due to the complexity of the disease etiology and diagnosis, repeated chest X-ray,CT and other senseless examinations, patients haven' t got effective treatment for a long time. These have brought a great deal distress to the diseased children' s study, life and psychology. They also lead to a serious waste of medical resources and increase the financial burden of diseased children.Chinese medical treatment for this disease has a certain advantage, but with no unified criteria for syndrome differentiation, treatment and efficacy.ObjectiveWe have concluded the clinical experience of Lin Jiwen,a pediatric director of Guangdong Provincial TCM Hospital. He uses a method to dredge the lung, regulate the spleen and remove sputum for wind-phlegm type chronic cough of children. With self controlled study design and Dr.Lin' s method, improvement of respiratory symptoms and TCM syndrome performance for wind-phlegm type chronic cough children were observed.Clinical efficacy, symptom improvement and safety of the method were objectively evaluated. This paper mainly talks about modern medical understanding and Chinese medical research of the disease, etiology and pathogenesis research, traditional Chinese and western medicine treatment, and so on.It is an overview of children' s chronic cough study at home and abroad.Knowing the physiological and pathological characteristics of children,we focused on the etiology and pathogenesis of Chinese medicine, summed up the cause of disease. Weakness of lung and spleen causes phlegm to block in lungs.Lung can' t dredge. Wind and phlegm combine together. Then cough stays long. Thus with the criteria of treating both superficiality and source, with the method to disperse wind, declare lung, stop cough, invigorate spleen and reduce phlegm,we have concluded the clinical experience of director Lin,who uses a method to dredge the lung, regulate the spleen and remove sputum for wind-phlegm type chronic cough of children.MethodTo observe the effect for Dr Lin' s method to treat wind-phlegm type chronic cough of children. With self controlled study design,150 children, who hadn' t taken any medicine or received any treatment for chronic cough in the past 2 weeks, were studied. They were treated by Dr Lin' s method for 4 weeks and were revisited 4 weeks after the treatment. The children' s cough extent, cough frequency, expectoration, nasal congestion, nasal discharge and other symptoms before and after treatment were observed.Results1.Comparison of the main symptoms:after treatment the major symptoms alleviated greatly and the difference was statistically significant(P<0.01).2.Comparison of effects:after treatment cough was significantly reduced and the difference was significant(P<0.01).3.Comparison of the overall effects:after treatment, the total symptom score showed that 49 cases were clinically cured,41 cases were markedly effective, 36 cases were effective and 1 case was ineffective.Total effective rate was 99.21%.4 weeks after the treatment, symptom score showed that 57 cases were clinically cured,39 cases were markedly effective,30 cases were effective and 1 case was ineffective.Total effective rate was 99.21%.Comparison of symptom score before and after treatment showed that the difference was statistically significant(P<0.01).Symptom score after treatment decreased significantly.ConclusionDr Lin' s method can significantly improved cough extent, cough frequency, expectoration, nasal congestion, nasal discharge and other symptoms of wind-phlegm type chronic cough children. This method could also significantly improve symptoms like phlegm hided caused by wind evil.It was really effective to dredge lung, stop cough, invigorate spleen and reduce phlegm,so it is very important for further guidance of TCM treatment of children' s chronic cough.
