| Objective To study the Therapeutic effect of external fixation for senior interrochantric fractures of Evans' typeⅢ.Methods According to Evans' classification,total 88 cases of old patients with Intertrochanteric fractures of typeⅢwere divided into two groups at random.44 cases were treated with close reduction and fixed with external fixator in the treatment group,and the other 44 with open reduction and fixed DHS internal fixation in the control group. Evaluation was made between the two groups in terms of fracture union, Harris hip score,complications during and after surgery.Results All the patients were followed up from 6 to]8 months,The average duration of hospitalization was 16.5±6.8 days in the external fixator(EF) group and 23.1±6.4 days in the Dynamic Hip Screw(DHS) group (P＜0.05).The average operative time was 32.9±6.8 minutes in the EF group and 71.7±13.0 minutes in the DHS group(P＜0.05).Statistically,there was no difference between preoperative hemoglobin and postoperative hemoglobin in the EF group(P＞0.05) but had significant difference in the DHS group(P＜0.05).The mean Harris hip score in six months was 89.0±4.6 in the EF group and 87.9±13.6 in the DHS group(P＞0.05).Reduction of collodiaphysial angle at six months was 2.9°±3.9°in the EF group and 3.3°±1.9°in the DHS group(P＞0.05).The time of clinical union was 12.2±1.3 weeks in EF group and 11.7±1.7 weeks in DHS group(P＞0.05),which had no significant difference.Conclusion For old patients with intertrochanteric fractures of Evans' typeⅢ,external fixator can provide similar fracture union rates,functional recovery of the hip and surgery complications to those of DHS internal fixation,whereas the external fixator is superior to DHS in terms of operative time,blood loss and hospitalization.It' s one of the ideal treatment methods for senior intertrochanteric fractures of Evans' typeⅢ. |
