ObjectiveThe aim of this study was to investigate the TCM syndrome patterns on postoperative fatigue patients throgh forward-looking survey of symptoms performance using data mining,and to provide a theoretical basis for the prevention of postoperative fatigue.MethodsIndexes such as symptoms and tongue-pulses were found through literature research,expert experience,Identification of TCM Syndrome Diagnosis(the second edition) chiefly edited by Yao Naili and Traditional Chinese Medicine Diagnostics(the fifth edition) chiefly editied by Deng Tietao.A symptom questionnaire consist of 74 indexes was formulated.The symptoms on the seventh postoperative day was collected and also the general conditions such as name, sex,VAS scores.Variables were entered into the database set up by SPSS13.0 software.The diagnostic criteria of TCM Syndrome was in accordance with Chinese medicine clinical syndrome-part of the terms issued by State Bureau of Technical Supervision in 1997,and Chinese medicine clinical research guiding principles released by Ministry of Health in 2002,combined with the clinical states.The syndrome types with its elements were explored by using principal component analysis,cluster analysis,discriminant analysis and combined with especiallist experience,the selection of the syndrome elements means the symptoms coming up at least two times in the three statistical results in our study.Results97 cases which fit the inclusion criteria were selected,which include 19 males and 78 females;The mean age was(45±13) years;33 cases accepted hepatic-gallbladder operation,31 cases accepted gastrointestinal operation, 20 cases accepted gynecological surgery,7 cases accepted thyroid operation and 6 cases accepted urinary tract operation.Five patterns of syndrome were generalized,i.e.spleen-kidney yang defficiency,splenogastric hygropyrexia, stagnation of liver-QI with deficiency of the spleen,liver kidney yin insufficiency and heart-spleen deficiency.The elements of spleen-kidney yang defficiency contains pale complexion,whitish sputum,aversion to cold and cold limbs,cold pain in waist,heaviness of limbs,tastelessness,loose stool,bulgy tongue,deep pulse.The elements of splenogastric hygropyrexia contains foul breath,pyrosis,acid regurgitation,general fever,abdominal distention,slippery pulse.The elements of stagnation of liver-QI with deficiency of the spleen contains emotional depression, irritability-restlessness,poor appetite,susceptibility to anxiety and preoccuption,pale tongue and whitish tongue fur,taut pulse.The elements of liver kidney yin insufficiency contains dull pain of hypochondriac pain,aching and cold weakness of waist and knees,tinnitus,night sweat.The elements of heart-spleen deficiency contains sallow complexion,lassitude,palpitation, amnesia,insomnia,dreamful sleep,dizziness,thin and weak pulse.ConclusionPostoperative fatigue is mainly concerned with dysfunction of spleen, kidney and liver,and most of the POF patients are suffered from asthenia-syndromes.The preoperative TCM syndrome types of patients who suffered from postoperative fatigue are mainly including spleen-kidney yang defficiency,splenogastric hygropyrexia,stagnation of liver-QI with deficiency of the spleen,liver kidney yin insufficiency and heart-spleen deficiency.It is a certain scientific method to study the TCM syndromes and its main including elements through combining use of principal component analysis,cluster analysis and discriminant analysis.
