Background Multiple sclerosis is an autoimmune disease characterized by inflammatory and degenerative disease of the central nervous system. The manifestations of its neurofunctional disorders are multiple attacks over time and space, and multiple sclerosis is one of the high disabled diseases.Studies on quality of life could date back to the 1930s,and have developed fast. The definition of quality of life according to WHO is that individuals of different cultures and value systems experience their goals, expectations, standards and concerns.Quality of life is combined of physical, psychological and social factors' evaluation norms, and scholars at home and abroad have increasingly placed emphasis on it.It has been widely used of all kinds of chronic diseases.Multiple sclerosis as one of the high disabled diseases, its pathogenetic conditions and therapeutic tools don't only affect patients' physical functions, but also affect patients' psychological and social functions. Therefore,it is important to introduce the concept of quality of life into the prevention and cure researches of multiple sclerosis, and adopt stable and effective test methods of multiple sclerosis, and explore factors that influence multiple sclerosis patients and continue to optimize curative effect evaluation system of multiple sclerosis. In China, there is no quality of life questionnaire of multiple sclerosis,we have acquired the written attorney of Professor Vickrey from America, and we apply and adapt Multiple Sclerosis Quality of Life(MSQOL)-54 measure, and evaluate the reliability and validity of this scale in Chinese version, and undertake preliminary clinical applications to get a effective instrument for assessing quality of life for patients with multiple sclerosis in China.Object To revise the multiple sclerosis quality of life-54 questionnaire in China and evaluate the reliability and validity of this scale and achieve accurate assessment of multiple sclerosis patients in China. Methods Using the English version MSQOL-54 as the original scale, through translation,back-translation,cross-cultural adaptation and protest we got the Chinese version of the scale. The subjects came from multiple sclerosis outpatients or inpatients of Nanfang Hospital from January 2007 to March 2009.Multiple sclerosis patients were chosen through on-the spot questionnaire. One or two weeks later, parts of them were asked to complete the same questionnaire again. All the data were analyzed with the statistic software SPSS 13.0 for social sciences. The reliability of retest was evaluated with Pearson's correlation coefficients;the reliability of internal consistency was evaluated with Cronbach's alpha coefficients;the content validity was evaluated with whether the research methods and procedures were standard or the content device is reasonable;the structure validity was evaluated with factor analysis;the criteria validity was evaluated by correlation coefficients;double sample test was done with Independent Sample T Test to test whether there would be significant total divided difference between two independent samples;and One-way ANOVA analysis was used to make comparisons of divided difference between multiple samples and comparisons between the divided differences within a sample.Results①This research formed Chinese version of MSQOL-54.②The analysis of the retest reliability of the scale showed the test-retest correlation coefficients of total scale and 13 aspects were above 0.7(P<0.05),except role limitations due to emotional problems aspect was slightly below 0.7(r=0.53,P=0.117).The internal consistency analysis showed that the Cronbach's alpha coefficients of total scales and 11 aspects were above 0.7,except health perceptions aspect was slightly below 0.7, demonstrating high internal reliability of the scale.③The evaluation of validity of this scale showed the construct validity was good as indicated by factor analysis; The correlation coefficient of criteria validity was 0.96(P=0.000);There were statistical differences in every aspect and total scale scores between research subject and healthy control subject(P<0.05).④The results of preliminary clinical applications showed Chinese multiple sclerosis patients scored low. There existed no significant difference between the scores in different sexes(P>0.05),except pain aspect; There existed significant differences in different age groups between the physical health, pain, health perceptions, health distress and overall quality of life aspects scores: physical health(t=- 2.405,P=0.021),pain(t=-3.758,P=0.001),health perception(t= -2.048,P=0.047),health distress(t=-2.563,P=0.014),overall quality of life(t=-2.372,P=0.022);There existed no significant difference between the scores in different marital status(P>0.05),except health distress and overall quality of life aspect; There existed no significant difference between the scores in different educational levels(P>0.05),except pain aspect; There existed no significant difference between the scores in different health insurances(P>0.05),except health perceptions aspect; There existed no significant difference between the scores in different disease durations(P>0.05);There existed no significant difference between the scores in different disease subtypes(P>0.05),except change in health aspect; There existed significant differences in different EDSS groups between the physical health, role limitations due to physical problems, and social function aspects scores: physical health(F=31.288,P=0.000),role limitations due to physical problems (F=13.534,P=0.000),social function(F=3.926,P=0.028).Conclusions1.The Chinese standard version of MSQOL-54 questionnaire has been formed by revising.2.The reliability and validity of Chinese version of MSQOL-54 questionnaire are good, and can be applied to assess quality of life for patients with multiple sclerosis in China.3.This research preliminarily suggests age and EDSS scores somewhat affect quality of life of multiple sclerosis patients. When evaluating the quality of life of multiple sclerosis patients, the influences of correlation factors must be taken into consideration.
