BackgroundHenoch-Scholein Purpura(HSP) is a allergic disease which can involve skin,joints, digestive tract,and kidneys,and with petechia and ecchymosis as its most frequent clinical manifestation.Its pathogenesis is now not completely understood,which leads to the dissatisfactions in treatment of this disease.Pathology of HSP prompts that transmigration of WBC to the vessels involved,and the activation of complement system may play very important roles in the pathogenesis of the disease,and those aspects are also the focus in the recent years.Anaphylatoxin C5a is one of the components in complement system,it can strongly attract WBC.This study try to explore the relationship between C5a and HSP by test the levels of C5a in patients with HSP and the controls.ObjectiveDetect peripheral blood level of C5a in HSP patients so that to know the relationship between anaphylatoxin C5a and HSP.Detect peripheral blood level of C5a in HSP patients before and after one week treatment to explore the relationship between C5a and the state of illness in patients with HSP.Methods30 patients with HSP(other immune system disorders and vasculitis are excluded) in our hospital were divided into group A(without digestive tract and/or kidneys involved),and group B(with digestive tract and/or kidneys involved),peripheral blood of those HSP patients before and after one week treatment was collected as study groups;peripheral blood of 20 patients with surgical diseases(immune system disorders and vasculitis are excluded) was collected as control group.Detect peripheral blood level of C5a by ELISA with Human Complement fragment 5a,C5a ELISA kit to see the alteration of the level of C5a in patients with HSP. Results1.Peripheral blood level of C5a in study group A and group B was significantly higher than that in control group(P<0.01);2.No significant difference of peripheral blood level of C5a was detected between group A and group B;3.After one week treatment,the level of C5a in the patients who are in remission phase was decended,while that in the patients who are in active phase was ascended. The variance of the C5a levels between the above two groups is statistically different (p<0.01).4.Levels of C5a detected in patients with HSP are still higher than that in healthy volunteers even when those patients are in remission phase after one week treatment.(p<0.01)ConclusionsThe levels of C5a are significantly higher in patients with HSP than those in healthy control group,and the activation of C5a is an important event related with the pathogenesis of HSP.No significant difference of peripheral blood level of C5a was detected between patients without digestive tract and/or kidneys involved and those with digestive tract and/or kidneys involved,we can not yet conclude that C5a is related with involvement of digestive tract or kidneys;further more,the levels of C5a in those patients modified as the activity of HSP modified,and C5a is relate to the activity of HSP;after one week treatment,peripheral blood level of C5a in those patients with HSP that were in remission phase were still higher than that in controls, C5a may be related with the recurrence of HSP.
