Objective TO investigate the variation of plasma endothelin(ET),Nitric Oxide(NO) in patients with Henoch-Schonlein purpura(HSP) and explore the correlationship between ET, NO and the lesion of kidney in HSP patients. Methods Plasma ET and NO were determined with RIA and Nitrate reductrate respectively in 51 cases with HSP and 30 normal controls, creatinine clearance rate(Ccr) were also measured with Beckman automatic biochemistry instrument made in USA and urinary protein excretion with urinary analyse method in these patients.Renal biopsies were performed and renal biopsy specimens were observed by light, immunofluorescence and electron microscopy in 12 out of 31 cases with Henoch-Schonlein purpura nephritis (HSPN). Results Comparaed with normal controls,the plasma level of ET and NO in patients with HSP increased(P<0.01).The level of ET and NO were higher in HSPN group than that in HSP patients without renal lesion(P<0.01). The level of plasma ET correlated positively with the level of plasma NO in HSP patients(P<0.01). The level of ET and NO conelated negtively with Ccr(r=-0.715,-0.721 ;P<0.01) and positively with renal pathologic grade(rs=0.866,0.777; P<0.01) in HSP patients;In HSP patients presented as nephritic syndrome,the level of ET and NO correlated positively with urinary protein excretion. Conclusion Plasma ET and NO may have an important role in the pathogenesis of HSP. There is a close correlation between ET,NO and the degree of renal histological injury. In some extent plasma ET and NO is significant in evaluating the prognosis of HSPN.
