Comparative Analysis Between Primary Gastric Lymphoma And Gastric Adenocarcinoma

Objective To evaluate differences of following parameters,clinical manifestation, mass location,endoscopic or morphologic classification,infection of Hp(Helicobacter pylori) and histopathologic features before and after the operation between primary gastric lymphoma(PGL) and gastric adenocarcinoma(GA),which can facilitate differential diagnosis and treatments.Meanwhile,it was to analyze the data of the clinical and histopathologic of the early primary gastric lymphoma to facilitate its early diagnosis and treatment.Methods Comparative analysis were focused on the clinical and histopathologic features of primary gastric lymphoma in 46 patients and gastric adenocarcinoma in 200 patients,such as age,course of diseases,symptoms,mass location,endoscopic or morphologic classification,infection of Hp and histopathologic features before and after the operation.Besides,clinical and histopathologic features was analyzed in 9 patients with early primary gastric lymphoma.Results Both PGL and GA patients had atypical clinical digestive manifestations of abdominal pain,abdominal distension,and so on.And they were also presented with melena.The mass of the PGL cases often located in corpora ventriculi and antrum (P＜0.05),and rare in fundus ventriculi and cardia,and endoscopic morphologic feature was observed with multiple ulcers or huge mass(Ⅱc lesions)(P＜0.05).On the contrary,the mass of the GA often located in antrum,and was solitary.The ratio of the infection of Hp in the PGL was higher than that in the GA patients(P＜0.05).So its veracity of dignosis before the operation was lower than that in the GA patients (P＜0.05).Conclusion PGL should be considered when there were mutiple erosion or ulcers includingⅡc lesion.So it is very important to improve the biopsy by gastroscope,from which the early differential diagnosis between the PGL and the GA can benefit, combined with the imageology examination,such as the endoscopic ultrasound,and guide clinicians for a better treatment of the diseases.And the early primary gastric lymphoma often appearedⅡc leision,and its prognosis was better with the treatment of surgery,.chemotherapy and Hp eradication.
