Objective:To investigate the effect of Biocoen on HIF1αexpression and the protective effects of Biocoen in rats after cardiopulmonary resuscitation(CPR).Methods:CA was induced by asphyxiation(succinylcholine) and ice-cold 0.5mol/L KCl in rats and resuscitation began five minutes later.In this experiment, seventy-two male SD rats were randomizly divided into 9 groups:control group (sham group),Conventional recovery 0.5h,2h,4h,6h(common treatment groups)groups,and Biocoen intervention 0.5h,2h,4h,6h groups(Biocoen treatment groups)(each group n=8).Western blotting was used to test hypoxia-inducible factor la protein expression of each group rat cardiomyocytes.The pathological changes of myocardium were observed under the optical and transmission electronic microscope (TEM).Result:Compared to the control group,there was a significant enhance of hypoxia-inducible factor 1a protein expression of myocardial cell in common CPR groups(P<0.01),the expression reach the peak at 2 hours after ROSC(0.57±0.06). After CPR,quantity of MDA significantly increased(P<0.01),meanwhile,enzyme activity of SOD and Na~+-K~+-ATPase activity declined significantly in myocardial cell compared to the control group(P<0.01).Na~+-K~+-ATPase activity reached the lowest values at 0.5 hour after restoration of spontaneous circulation(ROSC) (4.13±0.12μmol/mgprot/hour) and enzyme activity of SOD reached the lowest values at 2 hours after ROSC(6.35±0.84 U/mgprot).Compared to the common CPR groups,there was a reduce of HIF-1a protein expression of myocardial cell at different time in Biocoen intervention group,significant difference was found in the 2 hour groups(Biocoen intervention group 0.29±0.03,common CPR group 0.57±0.06,P<0.01).Compared to the common CPR groups,quantity of MDA significantly reduced and enzyme activity of SOD and Na~+-K~+-ATPase activity significantly increased in Biocoen treatment groups,but still under the normal level.Significant difference was found in Na~+-K~+-ATPase activity between the 0.5 hour groups(Biocoen intervention group 4.71±0.21μmol/mgprot/hour,common CPR group 4.13±0.12μmol/mgprot/hour P<0.01),and SOD activity between the 2 hour groups(Biocoen intervention group 8.36±1.37 U/mgprot,common CPR group 6.35±0.84 U/mgprot P<0.01).Transmission electron microscopy showed the myocardial cell damage performance in rats after CPR:nuclear deformation,marginated chromatin,the nuclear membrane rupture,pyknosis,mitochondrial swelling,ridge fracture or even disappear,matrix smeared out,part of the vacuoles,crude surfaced endoplasmic reticulum expansion;There were visible morphological,accompanied by inflammation and necrosis on myocardial tissue slice.Conclusion:Acute myocardial cells injury happened in rats after CPR because of hypoxia,ischemia-reperfusion injury et al.;Hypoxia inducible factor-1a protein expression enhanced in myocardial.Biocoen has the function of ameliorating the cellular energy metabolism,clearing the free radicals and making cell membrane and lysosome membrane stability;thus alleviating ischemia and hypoxia of myocardial; So that the HIF-1a protein expression reduced.Biocoen can reduced injury of myocardial cell due to ischemia-reperfusion(I/R),so it plays an important role in the protection of the heart after CPR.
