Nursing Needs And Correlative Factors Of Inpatients With AIDS

Objectives:1. To investigate the status of nursing needs, emotional feelings, family function, and social support in inpatients with AIDS.2.To analyze the influential factors of nursing needs in inpatients with AIDS.3.To explore the way to deal with inpatients with AIDS, so as to provide evidence for clinical nurses to work out more effective and targeted nursing interventions.Methods:1. Phenomenological method in qualitative research was adopted. 12 inpatients with AIDS were interviewed according to the semi-structured interviewing directions at some epidemic hospital in Fujian province from March to September in 2008 in order to explore the emotional feelings of inpatients with AIDS.2. 46 inpatients with AIDS from some epidemic hospital in Fujian province were taken with cluster sampling. The survey research last from July to December in 2008. Data were collected by the self-made personal characteristics questionnaire, APGAR, inpatients'nursing needs questionnaire and SSRS. The descriptive statistics, two-sample t-test, one way analysis of variance, Pearson correlation analysis were adopted. And a matched study was conducted. Drawing from the same epidemic hospital from July to December in 2008, an inpatient with AIDS was matched to two inpatients with HBV on the following variables: age, sex, time to be admitted to hospital, and inhabitancy. Datas were collected by the self-made personal characteristics questionnaire, APGAR, and SSRS.Results:1. The average score of nursing needs in inpatients with AIDS is 3.15 ±0.54 point, 58.7% of inpatients had high level of nursing needs, 39.1% of inpatients had moderate level of nursing needs, and 2.2% of inpatients had humble level of nursing needs. Among the five aspects of nursing needs, the score of physiological needs is the highest, whose score indicator was 80.55%. The score of love and belongningnee needs is second, whose score indicator was 80%.2. The emotional feelings of inpatients with AIDS concerned mainly about: feeling shocked when they got to know they had affected with AIDS; earnestly relying on family members; the need for mental supports; planning to go on to work after feeling better.3. The average score of family function in inpatients with AIDS is 5.80±3.63 point. There was a statistically significant difference compared with 8.60±1.74 point of HBV-inpatients. 28.3% of inpatients'family function was at serious disorder, and 21.7% was at moderate disorder.4. The average score of social support in inpatients with AIDS is 31.93±8.02 point, lower than the normal level(P<0.05) and the score getting by HBV-inpatients(P<0.01).5. There was a statistically significant relationship between physiological needs and marriage status, family function, social support (P<0.05). There was a statistically significant relationship between safety needs and social support(P=0.006). There was a statistically significant relationship between love and belongningness needs and social support(P=0.030). There was a statistically significant relationship between esteem needs and social support(P=0.011). And there was a statistically significant relationship between needs of self-realization and marriage status, family function, social support (P<0.05 or P<0.01).Conclusions:1. The nursing needs of inpatients with AIDS were at high and moderate level. Among the five aspects of nursing needs, the physiological needs were still primary. It is necessary to attach importance to and take measures to satisfy the physiological needs.2. AIDS raised havoc to patients in aspects of emotion, feeling, social ability, life style and family relationship, etc. Nurses should estimate the changes in emotional reactions of inpatients with AIDS properly, understand their life conditions, and help them to promote their life quality by providing holistic nursing care.3. The inpatients with AIDS got lower APGAR than HBV-inpatients and half AIDS-inpatients had deficiency in family function. Nurses should find out patients'family function and guide the family members to deal with patients properly.4. The inpatients with AIDS got less social support than HBV-inpatients and normal people. It shows that people are still horrified to AIDS. Nurses should strive for better social support so as to improve the quality of life of AIDS-inpatients.
