| Lung cancer has become the disease which mortality increases the most rapidly recent years. The patients of lung cancer with chemotherapy have become a special and major group in the society. Patients who suffer from malignancies will probably have got some physical symptoms and adverse psychological reactions because of the fear to tumors and chemotherapy. Anti-tumor drugs have obvious negative effect on the immune function besides the positive results . The research of literatures tells us that the early reaction of patients who received chemotherapy have great effects on physical syndromes produced in the latter stage. Therefore, it is very important to give active and effective psychological intervention prior to chemotherapy. There are rare reports about earlier and thorough intervention. The patients have got no time to adjust psychological status and correct the wrong recognition.60 post-operative patients were randomly divided into the control group and the experimental group. In the experimental group, we carried out mental nursing after the operation till the start of the second course of chemotherapy. The control group received routine care only. Data were collected by (questionnaire of general state of health, SAS, SDS, QLQ-C30) on the 7th day after operation, the day before the first and the second chemotherapy. At the same time, serum TNF-αlevel of 15 patients from each groups were tested in order to investigate the effect of mental intervention in reducing the anxiety, depression emotion, improving the quality of the life and immune function.Results:1.No statistical difference was found between the two groups in age, gender, marriage status,educational background and TNM classification.2. On the 7th post-operation day, SAS and SDS of two groups have got the highest score compare with the two chemotherapy, but there is no statistical difference in the two groups. There is difference before the first chemotherapy in SAS(tSAS=5.433,P<0.001) and SDS(tSDS=6.291,P<0.001)and the difference is more obvious before the second chemotherapy in the two groups.3.The quality of life shows the difference in two groups: general health status, somatic function, role function, emotional function, fatigue, nausea and vomit, short breath, lose of appetite.4.Immune function is measured by the detection of TNF-αlevel: the experimental group's serum TNF-αlevel is relatively equal to that on the 7th post-operatice day, while the control group's serum TNF-αlevel decreases, and it has statistical difference.Conclusion :1.Post-operative lung cancer patients have obvious anxiety and depression who need mental nursing badly.2. The mental intervention can reduce patients' anxiety and depression, improve their somatic function and emotional function significantly and can also ease the symptoms of fatigue, nausea and vomit, short breath and insomnia, enhance the immune function.3. Prophylactic mental intervention - such as recognition and behavior therapy was used in lung cancer patients who received chemotherapy. As a result, functions in quality of life such as general health status, somatic function, role function, emotional function, fatigue, nausea and vomit, short breath and lose of appetite were improved.4. After mental intervention, serum TNF-αlevel of the experimental group declined not obviously as the control group's serum TNF-αlevel , and has difference compared with that before chemotherapy. This illustrates the immune function of patients would be damaged in chemotherapy, and psychological intervention can ease it to some extent. |
