Objective:The hepatogenic ulcer was occurred on the base of chronic hepatic disease,especial hepatocirrhosis with portal hypertension,it has not yet attracted enough importance.And because of atypical of the disease,it was easy to confusion,such as chronic liver disease, cirrhosis,liver cancer.If it was facilitated esophageal hemorrhage bleeding,it was easy to confusion with gastric varices,In recent years,with the widespread application of endoscopic and endoscopic examination of the relaxation of the indications,Start to update this opinion. Collected from January 2002 to January 2008 in our hospital 74 cases of hepatocirrhosis ulcer,Randomly selected 56 cases of liver cirrhosis as control group,they were analyzed retrospectively in order to explore the clinical characteristics and the relevant factors of HU, for early identification and timely and reasonable treatment,improve the diagnosis and treatment of HU provided a basis.Methods:Retrospective analysis of 74 cases of hepatocirrhosis ulcer,observed epidemiology and general characteristics.Analysis of the relationship between the disease and the liver function grading,esophageal gastric varices and portal width.Multi-factor analysis of the relationship between the disease and the liver function grading,esophageal gastric varices and portal width.Analysis of the relationship between sex and ulcer.All inspections were completed in our hospital.Experimental data indicated that the mean±standard deviation,analysis of count data analysis using X~2 test,P<0.05 shows that the difference was statistically significant,multivariate analysis using logistic regression analysis.Results:74 cases of cirrhosis complicated with gastric ulcer,47 male,27 female(male: female= 1.74:1),aged 30~77,with an average age of 54.14±12.133,duodenal ulcer 35 (47.30%),30 cases of gastric ulcer(40.54%),composite ulcer 9 cases(12.16%),the incidence of ulcer was have a little significant seasonal,the number of cases in 6,7 months is more. Ulcer diameter<1.0cm 56 cases,accounting for 75.68%;diameter of 1.0-2.0cm16 cases, accounting for 21.62%,ulcer diameter>2.0cm 2 cases,accounting for 2.70%,pathology showed inflammation.35 cases of duodenal ulcer(47.30%,22 cases of duodenal wall, posterior wall of five cases,others eight eases),30 cases of gastric ulcer(40.54%,including 16 cases of gastric antrum,5 cases of gastric angle,gastric body 4 cases,others 5 cases), composite ulcer in 9 cases(12.16%) This set of data showed that:The cause of 55 cases of liver cirrhosis is hepatitis B(74.30%),hepatitis C 11 cases(14.90%),alcoholic liver disease 3 (4.05%),1 case of cryptogenic liver cirrhosis(1.35%),stasis cirrhosis 1(1.35%),cholestatic cirrhosis 1(1.35%),hepatitis B cirrhosis combined alcohol 2(2.70%).patients with abdominal pain in this group are 25 cases(accounting for 33.87%),in addition to abdominal distension,pantothenic acid,bleeding(vomit coffee-like materials,asphalt-like it) and other symptoms,the set of data in 74 cases of hemorrhage in 8 cases(10.81%).Others accounted for 59.46 percent.Child-Pugh classification in accordance with the standards,combined gastro-duodenal ulcer in cirrhotic A,B,C three-tier percentage respectively 20.27%,37.84%, 41.89%.Different classification of liver function and liver ulcer used chi-square test,P<0.05, the difference was statistically significant,that is,the set of data showed that the incidence of liver ulcer is related with the liver function.With the increase of liver function,duodenal ulcer is more.the relationship between the occurrence of sites of liver ulcer and different classification of liver function,P>0.05 the difference was not statistically significant.Based on the results of Endoscopic examination,74 cases of gastric varices without esophageal 17 cases(22.97%),mild 8 cases(10.81%),moderate 24 patients(32.43%),25 cases of severe (33.78%).Hepatogenic ulcer occurrence and the extent of esophageal gastric varices used chi-square test,P<0.05.There were significant differences.With the increase of varicose veins,liver ulcers increased.The degree of gastric varices esophageal and liver ulcer used chi-square test,P<0.05,the difference was statistically significant.The relationship between the width of portal and Hepatogenic ulcer used chi-square test,P>0.05.The relationship between Hepatitis B surface antigen positive and hepatogenic ulcer:57 cases of peptic ulcer with cirrhosis after hepatitis B including 47 cases of hepatitis B surface antigen positive. Multi-factor logistic regression analysis,we can see that the esophagus gastric varice is more important than the classification of liver function and the width.Gender and hepatogenic ulcer are related to each other.Conclusions:(1) HU patients mostly male,50~59 years old is more.The cause of HU is hepatitis B and C.(2) HU is found more duodenal ulcer,and many smaller ulcers.Clinical manifestation is atypical,no significant seasonal,the data showed that the number of cases is more in the month of 6.7.(3)Classification of liver function and the occurrence of liver ulcer is relevance.With the decline of liver function,the incidence of gastro-duodenal ulcer increased.With the increase of varicose veins,the incidence of ulcers increased.(4) Whether the degree of varicose veins and the severity are related to the occurrence of the ulcer.With the increase of varicose veins,liver ulcer increase the occurrence of cases.(5) Through multi-factor logistic regression analysis,we can see that the esophagus gastric varice is more important than the classification of liver function and the width.(6) Gender is related to the occurrence of liver ulcer,male maybe is the risk factor of the liver ulcer.
