Objective: To study the difference of accuracy and sensitivity between DR and CT in diagnosing early femoral head necrosis (FHN) in adults, and the difference of manifestation rate between DR and CT in many signs.Methods: 91 cases (157 hips) with early FHN in adult analyzed and compared in many signs between DR and CT, which had undergone DR and CT in the same period and proved by MRI, pathology or follow-up.Results:①In 91 cases (157 hips), 132 hips (84.1%) are diagnosed and 17 hips (10.8%)are suspected by CT and other 8 hips (5.1%) are negative . 104 hips (66.2%) are diagnosed and 32 hips (20.4%) are suspected by DR, and other 21 hips (13.4%) are negative. It has statistical significance in diagnosing and manifesting early FHN in adult between DR and CT(X~2 = 13.3733, 6.4206, p<0.05).②High-densitive sclerosis is manifested in 144 hips by CT and 109 hips by DR, bright saccule shape area in 34 hips by CT and 7 hips by DR, gas in 5 hips by CT and no hips by DR, the femoral head with little out of normal shape in 43 hips by CT and 23 hips by DR, calcification of femoral head ligaments in 7 hips by CT and no hips by DR, arthrosis tumefaction in 53 hips by CT and 32 hips by DR, enlargement of iliopsoas bursa in 4 hips by CT and no hips by DR, muscle atrophy in 81 hips by CT and 61 hips by DR. It has statistical significance in manifesting high-densitive sclerosis , bright saccule shape area, gas, the femoral head with little out of normal shape, calcification of femoral head ligaments, arthrosis tumefaction, enlargement of iliopsoas bursa, muscle atrophy in early adult FHN between CT and DR (X~2=4.0516~24.9239, p<0.05).Conclusion: CT diagnosing early FHN in accuracy and sensitivity is higher than DR. CT manifesting high-densitive sclerosis ,bright saccule shape area, gas, the femoral head with little out of normal shape, calcification of femoral head ligaments, arthrosis tumefaction ,enlargement of iliopsoas bursa, muscle atrophy is higher than DR.
