BackgroudCondyloma Acuminata is a common benign lesion,caused by infecting human papilloma virus to human genitals and the surrounding skin.In recent years,the incidence appears to a upward trend.Genital HPV infects with the proliferative basal cell through the skin around the anus and mucosal injury into the body,HPV-DNA exists in the form of chromosomes or by integrating into the host chromosomes,and starts early and late gene transcription translation with the diagonalization process,differentiation and maturation of keratinocytes,and assembly forming virus particles,with the propagation and spread of the virus particles,forming characteristic disease.Because the pathogenesis of CA is not entirely clear,There so far is not effective anti-viral treatment for CA,and not satisfactory solution to the recurrence.Different individuals have different the response to HPV infection,suggesting that immune status has a certain impact in the occurrence of condyloma acuminata,particularly the host cell immunity. At present,It widely recognized that change of cellular immune function closely relates to the occurrence of CA.The body can not build an effective immune response,becoming the main reason for HPV persistent infection;so to explore the immune mechanisms of condyloma acuminata has important theoretical and clinical significance.So far,Toll-like receptor(TLR) is the first recognition receptor of the inherent immunity,Through direct perception to microbial pathogen for defensing response.In TLR family,TLRI-11 have been found,and they play important roles in inherent immunity defense,and also can adjust adaptive immunity,becoming the bridge of connecting inherent immunity and adaptive immunity.TLR has very important impact on immune system,becoming new target for treating various infectious diseases.TLR3,TLR4,TLR7 and TLR9 are involved in the anti-virus inherent immune response;TLR3 identify viruse double-stranded RNA and the intermediates,double-stranded RNA of virus replication process; TLR4 identify bacterial lipopolysaccharide,inducing specific immune to promote removal of the virus;TLR7 identify single-chain RNA virus; TLR9 identify bacteria and virus genome DNA and synthetic CpG ODNs,and it has important role in the pathogenesis of anti-infection, anti-tumor,mediated cell apoptosis and autoimmune diseases.Activation of TLR3,TLR4,TLR7 and TLR9 mediates anti-virus immune responses by initiating myeloid differentiation factor 88(MyD88)-dependent signaling transduction pathway or MyD88-independent signaling transduction pathway(only TLR3 ),So they play important role in anti-viral aspect. Through clinical observation to patients,the change of TLRs with peripheral blood mononuclear cells can become a new perspective to objective response to immune status,thereby effectively destruction, removal of HPV-infected cells,for reaching the purpose to treat CA。Objective1.To observe the expressions of four toll-like receptors:TLR3, TLR4,TLR7 and TLR9;2.To further explore the role in the pathogenesis of condyloma acuminata and the relationship between TLR and the prognosis of condyloma acuminata;MethodsThe expression of TLR3,TLR4,TLR7 and TLR9 in peripheral blood mononuclear cells was detected by flow cytometry with two-colour immunofluorescent staining in 60 condyloma acuminatum patients(34 cases in primary,26 cases in recurrence) and 30 healthy controls.Results1.The expression of TLR4 and TLR9 with the primary and recurrent CA patients in peripheral blood mononuclear cells is significantly higher than those in the control group,and there are significant differences(P<0.05).2.The expression of TLR4 and TLR9 with the recurrent CA patients in peripheral blood mononuclear cells is significantly higher than those in the primary group,and there are significant differences(P<0.05).3.Peripheral blood mononuclear cells do not appear to express TLR3 in primary CA patients,recurrent CA patients and the control group,and there are no significant difference(P>0.05).4.Peripheral blood mononuclear cells do appear to significantly express TLR7 in primary CA patients,recurrent CA patients and the control group,and there are no significant difference(P>0.05).Conclusions1.This study detects Toll-like receptor expression in PBMC with condyloma acuminata in protein level;2.The expression of TLR4 and TLR9 in PBMC with condyloma acuminata is significantly increased,suggesting that TLR4 and TLR9 may relate to incidence of condyloma acuminata;3.The recurrence of Condyloma acuminata may relate to increased TLR4 and TLR9 expression;4.PBMC with condyloma acuminata patients and the control group does not appear to expression TLR3,so we consider the number of specimens may result in the pilot deviation,and whether TLR3 relates to the pathogenesis of condyloma acuminata remains to be further studied.5.The expression of TLR7 in PBMC with Condyloma acuminata and the control group is significantly increased,and whether TLR7 relates to the pathogenesis of condyloma acuminata remains to be further studied.6.The test provides a new perspective for us on the expression of Toll-like receptors and the pathogenesis of condyloma acuminata.
