Asymptomatic hyperuricema is associated with hypertension,vascular disease and metabolic syndrome.A lot of studies showed that increased serum uric acid level is an independent risk factor for chronic heart disease and chronic kidney disease.In this study,we examined the effect of fructose on the level of serum uric acid in SD rat and studied hyperuricema in the inpatients of 2006 in the forth affiliated hospital of Jinan medical university.This study is divided into two parts:(1) We observed the situation of hyperuricemia in the inpatients of 2006 in the forth affiliated hospital of Jinan medical university,and its relationship with other chronic disease or biochemical examination.(2):SD rat hyperuricemia model:SD rats were feeded with high fructose powder diet for 16 weeks,we examined the level of serum uric acid and its excretion in 24 hours urine.Chapter 1 Retrospective study of asymptomatic hyperuricemia in the hospitalized patients and its related diseaseObjective:Through a retrospective study,we examined all cases of hyperuricemia in hospitalized patients of 2006 in the forth affiliated hospital of Jinan medical university,to analyze the incidence,diagnosis rate,treatment intervention as well as its related factors.Method:A retrospective study was performed on the hospitalized patients of the forth affiliated hospital of Jinan medical university in 2006.①From 11833 hospitalized patients of 2006,we sampled 110 patients randomly.These 110 patients were divided into 2 groups:hyperuricemic patients and non hyperuricemic patients.We recorded the biochemical analysis and lifestyle including smoking and alcohol intaking.We also recorded the drug using and some chronic diseases.②All the patients diagnosed hyperuricemia of 2006 during hospitalization were selected.We recorded all data like before. With these two parts of data,we analyzed prevalence of hyperuricemia,the situation of diagnosis and intervention of hyperuricemia in the hospitalized patients.We also try to analyze the relationship between hyperuricemia and related diseases.Resuits:(1)Among the 110 patients,25 patients would be diagnosed hyperuricemia (man 14,female 11).The total incidence is 22.7%(with man23.7%and female21.6%respectively).Four out of 25 patients diagnosed with hyperuricemia got allopurinal treatment.No dietary intervention was performed.The incidence of hypertension,hyperlipidemia,chronic renal insufficiency,kidney stones in hyperuricemia patients is significantly higher than patients with normal uric acid(P<0.05).And hyperuricemia patients have a higher fasting plasma glucose and serum total cholesterol level(P<0.05).(2) In total 11833 hospitalized patients in 2006,we found that only 460 patients were diagnosed with hyperuricemia during hospitalization (female178,male 272).However,based on the incidence of 22.7%,we should have 2697 patients with hyperuricemia.In 178 female patients,67 patients were treated with drug-allopurinal(4.79%),and dietary intervention(low-purine diet) was performed in only 4 patients(0.29%). Only 19 patients reached at the target value after the intervention.In 272 male patients,115(8.99%) patients were treated with drugs,dietary intervention(low-purine diet) were 5 patients(0.39%),only 22 patients reached at the target value after the intervention.In this study,we also found that the serum uric acid level correlated with the incidence of hypertension, coronary heart disease,chronic renal insufficiency.Conclusion:The incidence of hyperuficemia in hospitalized patient population is high.It is associated with cardiovascular disease and chronic kidney disease. So it is necessary to prevent and treat asymptomatic hyperuricemia. However,in clinical practice,this situation did not get enough attention at present.Chapter 2 Fructose diet feeding induced hyeruricemia in SD ratsObjective:Observing fructose on the impact of serum uric acid level and renal dama-ge in experimental rats.Method:Twenty-four male Sprague-Dawley rats,divided into three groups randomly: control group:feeding with normal diet;experimental group A:feeding with ten percent-fructose powder diet prepared by ourselves;experimental group B:feeding with thirty percent-fructose powder diet prepared by ourselves; We collected the blood and 24 hours urine every two weeks and sacrificed all rats at sixteenth week.We examined the serum uric acid level,creatinine, blood urea nitrogen,total protein,albumin,triglyceride and total cholesterol. We also check the 24 hours excretion of uric acid,creatinine,urinary urea nitrogen,protein.Results:The body weight increased normally in all groups.The serum uric acid in experimental group A and B increased significantly,and at 16th week the mean level is little higher than control group(P>0.05);there is no difference among the three groups.According to the study,the 24 hours uric acid excretion is higher in experimental group A and B than control rats at 16th week.The serum total protein in experimental group A and B is higher than control group(P<0.05),but there were no difference between experimental group A and group B.The serum total cholesterol in two experimental groups is higher than control group(P<0.05).The total triglyceride level decreased gradually in all three groups.Conclusion:Adding high fructose in the rat diet is safe for rat growing.The fructose diet can cause an increased production of uric acid and metabolic disturbance of cholesterol in rats.This rats can serve as a animal model of hyperuricemia and metabolic syndrome.
