The meniscus, a fibrocartilageous tissue found between the femur and tiba, it is responsible for shock absorbtion, load transmission, and stability within the knee joint.Damage to this tissue can lead to osteoarthritic changes, suggesting that the meniscus protect the knee joint from degenerative joint disease.Historically repair technique consisted of exision or suturing of the damaged tissue.Unfortunately neither of these techniques successfully repairs damaged tissue. tissue engineering is one possible solution.Object:To Preliminary study the effect of fibrochondrocyte-fibrin glue construct on the healing of meniscus injury in menscus avascular portion.Methods:1. To extricate fibrochondrocyte from rabbit menisci, then expand in vitro. study the biologic behavior. 2. To synthesize fibrin glue and fibrochondrocytes were added into the fibrin glue forming the cell-fibringlue construct. Then meniscus injury in avascular portion were performed on both knees of eight chinese white rabbits which been divided to three groups at random.After the rabbits had been anaesthetized with Nembutal via rabbit ear vein at a dose of 30mg/kg, an anteromedial incision was done on the medial side of both knee to open the skin along the medial superior border of patella to tibia tuberosity. The meniscus hock was used to expose the anterior horn and somatic part of medial meniscus as well as the cartilage surface of femoral condyle. transfixion pin were used to produce several full-thickness penetrating wound.Eight adult rabbit were prepare into the injuryed model in avascular portion of meniscus with full-thickness penetrating wound. Select four of the eight rabbits as A group,two of the rabbits as B group, and two of the rabbits as C group. In A group cell-fibringlue construct were implanted in laceration;in B group, single fibrin glue was implanted in laceration; and C group no any filler was applied for management.eight rabbits were sacrificed in the 4th and 6th weeks after operation.All menisci samples were collected and were placed in Formaldehyde solution for one week.Afterwards,with dehydration, paraffin soaking and embedding of sample, HE and Immunohistochemical staining was given for optic microscopic examination.Results:1. Meniscus fibrocartilage cells in primary culture: the first inoculate cells are the small rounded, suspended cells, beginning to adhere to the wall at 12-24 hours after inoculation. After 48 hours, most of cells adhere to the wall. The cells begin to elongate, and the majority of cells are angular, a small number of oval-shaped. With the increase in the number of cells, polygonal or oval cells gradually integrated to form single-layer,"paving stone"-like appearance. The growth of subculture cell was significantly quick than the original generation, and the same cell density cultivation of subculture cell begin to adhere to the wall at 12-24 hours after inoculation. Polygonal or oval-shaped passage cell gradually turn into a spindle or spindle length shape and cell volume enlarge. With connections between cells, the passage cell can covered with culture flask in 4-5 days. With the increase in the number of generations, the proliferation rate began to slow down。2. The effect of fibrochondrocyte- fibrin glue construct on the healing of meniscus injury in menscus avascular portion. 4 weeks after operation histological examination showed that: In A group, the defects were filled by meniseus-like tissue and few cartilage cells were observed occasionally; Group B meniscus tears were filled with fibrous tissue, There is no cell observed; In blank control group (C group), 4 to 6 weeks after operation, no any defect was observed to heal among the menisci and there were no chondrocyte proliferation presented on the border of defect. In A group,the defects were filled by meniseus-like tissue that not closely adhered to the adjacent tissue and a large amount of cartilage cells and collagenous fibrils were visible, Fibrochondrocytes were survived in reticular structure of fibrin.In group B, 6 weeks after operation, the fibrous tissue partial or completely absorbed, There is no cell observed.Conclusion:Fibrochondrocyte grow, proliferate vigorously in vitro and the fibrochondrocyte from meniscus tissue can be used as the source of meniscus tissue engineering seeding cell. fibrochondrocyte- fibrin glue construct has the effect of improve the healing of injury meniscus。...
