Objective:1.To have a knowledge of the current situation of nurse-patient relationship of Grade III-A Hospitals in Changchun City through the investigation and analysis of the problem of nurse-patient relationship in four Grade III-A Hospitals .2.To investigate and analyze the influence factors of nurse-patient relationship in Grade III-A Hospitals.3.To provide Countermeasures in order to improve the nurse-patient relationship in Grade III-A Hospitals.Method:1. Sampling Random sampling method, 380 nurses and 380 patients of four Grade III-A Hospitals in Changchun City were sampled randomly as the subjects. Totally 380 nurses and 380 patients were visited and 348 and 342 valid pieces of questionnaires was obtained, 91.6% and 90.0% validly.2.Design of questionnaires The questionnaire was self-designed and based on a lot of literatures and used questionnaires.The main questionnaire includes five Dimensions.Through pre-investigation and experts assessment , the reliability and validity of the questionnaire were good.3.Statistical methods SPSS13.0 for windows software was applied for data processing. Statistical methods include frequency analysis, Chi-square test and Rank sum test.α=0.05.Result:1.The Overall understanding of nurse-patient relationship In nurses and patients'opinion the Overall understanding of nurse-patient relationship is Lack of harmony. Patients at their own expense and Commercial insurance tension with nurses higher than others(P<0.05).The scores of 76.9% of the patients believe that nurse-patient relationship is Service and Customer. The scores of 88.6% of Patients at their own expense世界卫生组织hold this view is the highest(P<0.05).Tensions of nurse-patient relationship is severe . The scores of 88.5% of nurses have experienced nurse-patient disputes. 77.9% of nurses have experienced hospital violence. The scores of 89.6% of treatment nurse have experienced nurse-patient disputes and is the highest of all(P<0.05). The scores of 86.7% of office nurse have experienced hospital violence and is the highest of all(P<0.05).2. The satisfactory degree and needs of nursing services The satisfactory degree of nursing services followed by high to low are nursing skills, admitted reception, overall satisfaction with care, service attitude, Interpretation and health guidance. The biggest need of nursing services is improving service attitude,next is high level nursing,then Clean and comfortable hospital environment. The scores of 96.8% of nurses hold that the need of nursing services is improving.3.Lower vocational identityDue to the results of status, life status, psychological pressure and job evaluation of nurses , the vocational identity of nurses is lower than before. As primary nurse , the scores of 79.3% of them take care of more than five patients per day. All of the nurses take care for more than thirty patients per day. The scores of 65.2% of nurses work for more than nine hours per day. The scores of 92.2% of nurses have experienced an emergency recall during rest time.They choose sleeping,leisure and doing housework as the most important thing during rest time. The scores of 80.2% of nurses'psychological pressure is great, treatment nurses and temporary nurses'psychological pressure is the most serious(P<0.05). The scores of 98.9% of nurses not hope their children to be a nurse, The scores of 94.3% of nurses believe nursing is high-risk. The lowest job evaluation come from treatment nurses and temporary nurses(P< 0.05).4. Status of nurse-patient communicationThe scores of 80.1% of patients feel that the first impression of nurses is general. 75.7% of them get along well with nurses between 30-40 years old. 64.9% of them trust of nurses in medium level. 74% of them think that nurses get along medium with their relatives. Status of nurse-patient communication is unsatisfactory.5.The focal point for medical treatmentThe scores of 40.2%% of nurses agree with burden of proof,but the proportion of patients is 50.9%(P<0.05). The scores of 87.1%of patients agree with media reports, but the proportion of nurses is just 0.3% (P<0.05). The scores of 94.5% of patients think that the care costs is at a high level , but 51.7% of nurses think that the care costs is at a low level (P<0.05).A large number of graduates, junior college students and undergraduates agree with burden of proof is higher than others,but the proportion of disagree mainly in technical school students and senior high school students(P<0.05). A large number of public patients agree with supervising nurses ,many new rural cooperative medical care of patients disfavour(P<0.05).6. Influencing factors of nurse-patient relationshipThe scores of 91.4% of nurses consider that medical health system is the most important factor to impact nurse-patient relationship. The scores of 97.1%of patients consider that service attitude is the most important factor to impact nurse-patient relationship.Conclusions:1.The current situation of nurse-patient relationship is lack of harmony in four Grade III-A Hospitals in Changchun City, and need improving further.2.The most important objective factor to impact nurse-patient relationship is medical health system, subjective factor is service attitude.3.The subjective attitude and behavior of nurses and patients are all affected by medical health system.4. The Grade III-A Hospitals in Changchun City should follow the new policy of Medical Health System Rerorm to improve hospital soft environment, attention to the quality of training of nursing staff, improve hospitalize behavior of patients and strengthen communication with the media to improve the current nurse-patient relationship.
