| Objectives:The number of women used LNG-IUS is more and more in China.Many foreign studies show that the satisfied level of using LNG-IUS is better and advance information improves user satisfaction.In recent years,many internal studies involving connected problems but special study on that is few.This study is aim at understand user satisfaction and consultation on using LNG-IUS and its influential factors.The specific objectives include①the status of using LNG-IUS;②user satisfaction of LNG-IUS and its influential factors;③consultation of LNG-IUS and its influential factors;④advance information improves user satisfaction.Methods:This study included quantitative study and ln-Deepth Interview(IDI).We selected the women who used LNG-IUS in the past five years and were detailed registered in mirena company as subjects.The quantitative study is implemented by phone with closed-questionnaire.The investigator had been trained.And every questionnaire had been checked by investigator,quality controller.The software EpiData3.0 was used to found the database,and two special workers had put the data into the computers separately.Data was analyzed by the descriptive statistics and the Chi-square test with SPSS 13.0 statistic software.We had made the interview instrument,and trained the interviewers.We found 20 women who had experienced domestic violence from the baseline investigation,and had the informed consent of the women to record the interview process with the record pen.We transcribed and coded the data,listed the sorting form,and wrote the report.Results:It included four parts:status of using,satisfaction and consultation of LNG-IUS and effect to satisfaction by consultation.Every part included the results of quantitative study and IDI.1.Using status:①before using,81.6%of user understood the information on LNG-IUS from hospital or doctor and 45.7%of them used LNG-IUS because of recommendation of doctor.In the IDI,many women expressed the similar situation,which include "the doctor introduced it","the doctor said that is better" and "I saw the propaganda column about it in hospital".②After using,93.9%of user suffered decreasing menstruation.84.0%of women suffered dysmenorrhea whose symptom was eased.In the IDI,many women expressed the similar situation,which include "the menstruation is decreased and the period is shorted" and "the dysmenorrheal is relieved".50.5%of them suffered adverse reactions,and 62.6%of which had a bit bleeding.The situation of IDI includes "I have a bit bleeding" and "bleeding and gaining flesh".③41.4%of users had absence of menstruation.The connected study shows that the satisfied level of women who got the information on absence of menstruation was better than that of women who did not get the information on that(χ2=5.073,P＜0.024). That level of women who got the information on the menstruation is resumed once stop to using LNG-IUS was better than that of women who did not get the information on thatχ2=4.585,P＜0.032).And the level of women whose attitude to absence of menstruation was "happy" and "not to be good or bad" was better than that of women whose attitude to absence of menstruation was "worry" and "not to be accept"(χ2=74.988,P＜0.000).The situation of IDI includes "no menstruation several months after using it","I am not worry about it because the doctor said something about it tome" and "I trust the doctor,she said menstruation will come after stop to using it".④9.0%of users had terminated using LNG-IUS and the proportion of which the reason was unsatisfied was 6.12%of them.The special reason include bleeding(2.52%),absence of menstruation(1.79%) and reaction of hormone(1.80%).The study on influential factors show that the rate of terminated using, which the users were satisfied with the follow-up(χ2=8.167,P＜0.017),the effect on daily life was small(χ2=34.067,P＜0.000) and the adverse reaction was slighter(χ2=9.742, P＜0.002),was lower.2.satisfaction:The status of satisfaction of LNG-IUS was better and the rate of that was 88.5%.the satisfied rate on contraceptive effect was the highest(98.2%).The unsatisfied rate of price was the highest(31.8%),and the follow was adverse reaction (24.1%) and absence of menstruation(20.2%).the situation of IDI include that "the contraceptive effect is better","it is more expensive","the bleeding is last several months, it is uncomfortable" and "I have not the menstruation,it is strange".The study show that if the women was younger than 35(χ2=87.677,P＜0.000),had normal period(χ2= 14.080, P＜0.000),no adverse reaction(χ2=ll.312,P＜0.001 ),long using time(χ2=5.319,P＜0.001 ), had consulted before using(χ2=5.198,P＜0.027),had got a better attitude of consultation (χ2=7.767,P＜0.014) and satisfied answer(χ2=8.931,P＜0.012) from doctor,the satisfied rate on using LNG-IUS of them was better. 3.consultation:There were 36.6%of user had consulted before using LNG-IUS and 79.7%of them consulted doctor.Among them,48.2%answered all questions and 88.9%of users were satisfied at the answers.There were 49.8%of user had consulted before using LNG-IUS and 82.7%of them consulted doctor.84.1%of users were satisfied at the answers. In the IDI,some subjects expressed that "no consultation,use it only because the doctor said it is good","she(the doctor) is busy and say nothing to me" and "I have consultation in hospital".The study show that if the users was younger(χ2=16.632,P＜0.001 ),had high income(χ2=8.053,P＜0.018) and used LNG-IUS because of contraception(χ2=5.989, P＜0.014),treatment(χ2=15.763,P＜0.000) and slight adverse reaction(χ2=7.485,P＜0.006), the consultation rate of them was higher,and if the users had abortion history(χ2= 12.274, P＜0.001),understood information on LNG-IUS from hospital(χ2=22.765,P＜0.000) and use OC before using LNG-IUS(χ2=8.538,P＜0.003),the rate was lower.4.effect to satisfaction by consultation:The study show that if the women had consulted before using(χ2=5.198,P＜0.027),had got a better attitude of consultation (χ2=7.767,P＜0.014) and satisfied answer(χ2=8.931,P＜0.012) from doctor,the satisfied rate on using LNG-IUS of them was better.In the IDI,some women said "if the doctor said something about it,I were not so anxious about it","the doctor have told me these things before I used it,so I am not worry about it now" and "I asked the doctor it when I found I have no menstruation,she explained that to me patiently,so I am not worry".Conclusions:1.The most information on LNG-IUS was from hospital and doctor.Recommendation of doctor was important to using LNG-IUS for users.After using it,half of users had adverse reaction and two fifths had absence of menstruation.Advanced information could improve user satisfaction on absence of menstruation.And slight adverse reaction,small effect on daily life and follow-up in time,which were all could decrease the rate of terminated using.2.The user satisfaction was better,especially that of contraception.If the users were younger,had normal period after using it,no adverse reaction,long using time,advanced information and got a satisfying answer and kind attitude from doctor,the user satisfaction was better.3.The consultation on LNG-IUS was not good.Only one third of users got advanced information and about half of them had consultation after using it.Most of consultation providers were doctors.If the users were younger,had high income,and used LNG-IUS because of contraception,treatment and slight adverse reaction,the consultation rate of them was higher,and if the users had abortion history,understood information on LNG-IUS from hospital and use OC before using LNG-IUS,the rate was lower.4.A good consultation on LNG-IUS could improve user satisfaction.A full and timely consultation could increase the understanding on LNG-IUS of users.So the users were good prepared for possible change of physiology and psychology,alleviated the related worry and panic,increased trust of using LNG-IUS and then improve user satisfaction.
