ObjectiveTo investigate the techniques of the total hip arthroplasty in the treatment of Ankylosing Spondylitis(AS) with tetanic deformity of the hip.Patients and Methods1.Patients:38 patients with tetanic deformity of the bilateral hip to AS were performed with the total hip arthroplasty from October 2001 to December 2007.Among the patients,28 were male and 10 were female,with their ages ranging from 25 to 56 years and an average age of 38 years old.All patients fulfilled the diagnostic criteria of AS(Roman criteria of AS,1963).The Harris scores of preoperation were 9-64,mean 36.6.2.Method:total hip arthroplasty was performed on bilateral hip at the same time with a lateral approach.Antiseptic treatment and systemic healing exercise was regular after operation.With a follow-up for 6-30 months,therapeutic effect was evaluated by Harris grade system and X-ray examination.ResultsA follow-up was taken for 6-30 months(mean 18 months).Deep venous thrombosis and injury of nerve and vessels were not found.No dislocation,aseptic loosening and fracture of prosthesis.The pain was eased after operation.The pre-operational movement range of all the hips in this group was 0°The post-operational movement range was significantly improved.The range of bending and extending was 50-100°(mean 79.5°),while the adduction range was 10-15° (mean 12.9°),and the abduction range was 15-35°(mean 22.7°).Last score by Harris grade system was 56-92(mean 81.2).ConclusionTotal Hip Arthroplasty was effective to treat the patients with ankylosing spondylitis.THA can significantly rectify the deformity of tip and improve the function.The effect on these cases is encouraging.Almost all the patients with AS were satisfied with the operational outcome,which used careful pro-operational plan, correct operational rebuilding and active post-operational healing exercise.
