Backgroud China's HIV/AIDS epidemic has entered in a rapid increase stage. Venous injection drug using is still the main transmission route of HIV in China. While the percentage of the route of the heterosexual transmission of HIV is rapidly increase.Now HIV is spread from the high risk population to the general population. Current research showed miners have become one kind of the bridge population who spread HIV from the high risk population to the general population and one of the promote factors in transmitted HIV.The incidence of HIV and other sexually transmitted diseases(STDs).the prevalence of HIV/STDs.the trend of HIV/STDs and other information is very useful for establishing and carrying out the effective HIV prevention and control strategies and measures.Objectives To know about the situation of the HIV/STDs prevalence in miners of Gejiu City,Yunnan province;the influencial factors of miners infected HIV/STDs and the change trends of miners'HIV/STDs infection and behaviour characteristics.In order to make the aimed HIV/AIDS health education in such area of Yunnan, supplying the significative and reference materials and also building the foundation for establishing the intervention strategies and mearsures.Methods and materials In December 2005-January 2006 and August-November 2006,the repeated cross-sectional epidemiology surveys were carried out in miners of Gejiu city,Yunnan province.Standard questionnaires were used in the repeated cross-sectional studies to collect social background information,drug use history, general sexual behaviors and commercial sexual behaviors and other HIV/AIDS telated high risk behaviors.In the repeated cross-sectional studies,the biological samples of venous blood,urine were collected individually from the participanted miners.Laborotary test were carried out in the two stage cross-sectional studies to antibodies of HIV-1,HSV-2,syphilis,Urine morphine,CT and NG.Results In the two stage cross-sectional studies,there are separately 526 and 533 miners enrolled in the study.In which,72 miners attended the two cross-sectional studies,which accounted for 13.69%(72/526) and 13.51%(72/533).In the two stage cross-sectional studies,the rate of HIV infection in miners who were self-reported having sex was from 0.71%(3/421) down to 0.25%(91/401).The changes of prevalence for other STDs were as follows:syphilis 1.43%-1.25%,HSV-2 11.88%-11.22%,CT 5.46%-6.49%,and NG 0.24%-2.16%,STDs(include HIV) 17.39%-15.46%.By Logistic analysis,it shows the follow factors are the related influential factors of infected STDs:in the first stage cross-sectional studies,it shows self-reported having drugs(OR=0.25,95%CI:0.05-1.36),multiple sexual partners (OR=0.50,95%CI:0.26-0.95) and the miner whose genitals ever had pain or uncomfortable(OR=0.42,95%CI:0.18-0.99) are the hazard factors of STDs infection; the un-Honghe prefecture miners(OR=0.61,95%CI:0.33-1.12) is the protect factor of STDs infection.In the second stage cross-sectional survey shows the first sex age down to 20(OR=0.51,95%CI:0.26-1.00),having sex with FSWs (OR=2.44,95%CI:0.96-6.20) and(OR=0.49,95%CI:0.24-1.00) are the hazard factors of infected STDs.And un-the Honghe miners (OR=0.53,95%CI:0.28-1.03),the miners who ever worked in the 3 or more cities (OR=0.24,95%CI:0.09-0.63) are the protect factors of infected STDs.The influential factors of infected HSV-2:in the first stage survey, the minority(OR=2.15,95%CI:0.96-4.80),marriage(OR=2.83, 95%CI:1.27-6.31),the miner whose genitals ever had pain or uncomfortable (OR=0.37,95%CI:0.15-0.95),multiple sexual partners(OR=0.39, 95%CI:0.17-0.90),self-reported having drugs(OR=0.22,95%CI:0.03-1.41) and the high rate of knowing STDs/HIV knowledge(OR=2.23,95%CI:1.00-4.97) are the hazard factors of infected HSV-2.in the second stage survey, having sex with FSWs(OR=2.87,95%CI:0.91-9.05),month salary high over 1000(OR=2.12,95%CI:0.95-4.70) and the low rate of knowing STDs/HIV knowledge(OR=0.46,95%CI:0.17-1.22) are the hazard factors of infected HSV-2.The first sex age over20(include 20)(OR=0.40,95%CI:0.20-0.80),the job is temporary and floating miner(OR=0.27,95%CI:0.12-0.60) the miners who ever worked in the 3 or more cities(OR=0.27,95%CI:0.09-0.81) are the protect factors of infected STDs.In the two stage cross-sectional studies,the correct answer rate of STDs/HIV knowledge in miners is dividedly 37.56%(4111/10946) and 37.12% (3870/10426).The main source and the front-two mode of STDs/AIDS knowledge acquirement are TV and talking with friends.Conclusion The miners in Gejiu city have high rate of STDs infection,low knowing rate of HIV/STDs knowledge,and most of surveyed miners existed high risk sexual behaviors,such as the rate of using condoms,prevalent multiple sexual partners,high rate of having commercial sexual behavior,and the serious epidemic situation in Gejiu and so on.These phenomenon clew miners should be as the emphasis population and high risk bridge population in our future HIV/AIDS prevention and control work.And strengthen the health education of Gejiu miners' HIV/STDs knowledge and behavior intervention,as to enhance the knowing rate of miners' HIV/STDs knowledge and reinforce the miners' hazard consciousness.Meanwhile,in order to promote the effection of AIDS prevention and control,we should develop aimed behavior intervention,emphasis on free fee provide condoms,and improve condom's getable and using rate,to change the mode of hazard behavior.Moreover, supplying the canonical service of STDs diagnosis and treatment to increase the STDs infected rate in miners and to control and cut down the STDs/HIV transmitted and epidemic speed.As to strengthen the effection of current HIV/AIDS prevention and control and promote HIV/AIDS prevention and control.
