Objective: To research the influencing factors of internal fixation failure and postoperative fractures nonunion. Methods: From Feb 2005 to Oct 2007, 38 patients were treated in Shandong University of TCM Affilliated Hospital Orthopaedics Patients'Room who showed up internal fixation failure and postoperative fractures nonunion. 16 cases of all belonged to internal fixation failure were treated by replacing internal fixation plus bone grafting. 22 cases came from postoperative fractures nonunion, 10 of which were treated by reserving original fixity and using simple bone grafting,12 of which by replacing internal fixation plus bone grafting. Postoperative treatment effectiveness was evaluated according to fractures bone union evaluation criteria. Results: 38 cases were all cured, and all came to bone union. The time of bone union was from 3 months to 6 months (mean 3.8 months). Conclusion: Iatrogenic factors (factures malreduction, bone defect, improper selection of the internal fixation materials, wound infection) and patients'incorrectness of functional exercises are two causes of internal fixation failure and postoperative fractures nonunion. Modus operandi to restart fracture healing is stabilizing fractures extremitates plus bone grafting.
