Objective To investigate the clinical characteristic and treatment analysis of chondrosarcoma of soft tissues. Methods A series of 7 patients with chondrosarcoma of soft tissues (mesenchymal chondrosarcoma 4 cases and myxoid chondrosarcoma 3 cases) treated in Qilu Hospital affiliated to Shandong University between 1988 and 2007 is analyzed retrospectively. To investigate the clinical characteristic and treatment and prognosis of soft tissue chondrosarcoma combining with internal and overseas literature.Resules This group included 7 patients, 3 cases were male and 4 cases were female; the age of onset is between 33 and 70 year-old and the average age is 45.7 year-old; the part of onset: 4 cases were thigh and 3 cases were leg. The case history is from 10 months to 5 years. The frequent clinical appearance was local tumor and functional disturbance which nearing to joint. The show of X-ray was clear bouncary and orbicular-ovate soft tissue tumor with a great quantity irregularity and punctiform and flocc calcific. 2 cases adjacent skeletal bones were eroded; some calcification can be showen on CT. Amputation surgery 3 cases, partly enlarged excision 4 cases, postoperation radiotherapy 3 cases, generalize chemotherapy 2 cases, the rest 2 cases can't march any adjunctive therapy because of economic reason. Postoperative pathological resules showed: 4 cases mesenchymal chondrosarcoma and 3 cases myxoid chondrosarcoma. Appearances in naked eye showed that mesenchymal chondrosarcoma was uncomplete pseudo-capsule, canous cross section, dispergented translucent cartilage and calcification; myxoid chondrosarcoma was soft, sharply marginated peplos, canous myxoid or tremelloid cross section. The histological characteristic features showed: mesenchymal chondrosarcoma was alternative composed by two elementary cell components which were undifferentiated mesenchymal cells together with islands of chondrosarcoma, without segmented structure, the demarcations between mesenchymal cells and islands of chondrosarcoma were limpid. Myxoid chondrosarcoma cells were arranged according to himantoid and ascidial alinement. There were a great quantity myxoid base material around lepto volume neoplastic cell with acidophilia endochylemas and conspicuous nucleoles and minor hyaline cartilage. In follow up period from half a year to 9 years (average 3 years), 1 case had been put on amputation surgery on the quarrus year after local excision and has survivaled 9 years without sarcoma; 2 cases have survivaled half a year without sarcoma postoperation; the result 4 cases succumbed to pulmonary metastasis on the first or the 1.5 year postoperation. Conclusion Chondrosarcoma of soft tissues is rare tumor which genesises soft tissues, difficulty to diagnose, and its attack rate is uncertain. The patients of chondrosarcoma of soft tissues is preferente middle-aged, the lower limbs, especially thighs and legs etc. The main treatment method is exairesis, amputation surgery and partly enlarged excision, radiotherapy and chemotherapy to assist postoperation. The follow up time should be long because palindromia and metastasis always occurrence after several years postopeeration.
