Objective: The incidence rate of ischemic cerebrovascular disease tend to increase every year,especially among young people.It's pressing to find its causes and promotion circumstances and then work on to prevention and cure this disease. Artherosclerosis(AS)is the main foundation of pathology in cerebral infarction. Recent studies show that infection of microbes,especially human cytomegalovirus (HCMV) is relavant to AS. Inflammation and immune reaction caused by activated infection of HCMV plays an important role in the development process of AS. The main of this article is to investigate the relationship between carotid artery artherosclerosis and activated infection of HCMV,E-selectin,matrix metalloproteinase 1(MMP1).Method:1 The investigation subjects were 141 cases of patients with ischemic cerebrovascular disease and 27 healthy persons,all of the cases conduct ultrasound of arteria carotid,HCMV-PP65 of peripheral blood leucocyte,E-selectin and MMP1. Compare the differences of HCMV-PP65,E-selectin and MMP1 in the patients with different kinds of plaques, further to compare the differences of E-selectin,MMP1 between PP65 positive and negative paitents.2 Ultrasound examination method: Use color Doppler ultrasonograph of HDI ATL 5000 producted by American, with linear array detecting head, frequency of waves is 5~12MHZ.With dorsal position, detecte from originate of arteria carotis to earteria carotis interna when entered the skull.Examine both of the arteria carotis use consecutive T-sect,longitudinal plane 2D gray-scale and frequency spectrum ultrasonic examination by Doppler's method. Review and record the IMT,mainly observe whether exist atherosclerotic plaque or not, the position,size,resonance,surface characteristic of atherosclerotic plaque.3 Detect HCMV-PP65 antigen in peripheral blood leucocyte use the method of immunohistochemical; Assaying MMP-1 and E- selectin in peripheral blood use the metod of biocatalyst connected immune adsorption double antibody sandwich.4 The various plaques were grouped into 6 types on the basis of the course of disease development, kinds of pathologic changes and Hennerici classification. For the statistical convenience, the patients with soft and flat plaque were included into instability plaque group, the patients with endometrial hyperplasia and hard plaque were classified into hard plaque group. Statistics in the principle of double-blind method according to the report and photograph.Results:1 The positive rate of HCMV-PP65 was higher in Case group than the control,it is 48.9% to 0%.2 Different plaques have different HCMV-PP65 postive rate, in the instable plaques is 59.48%,it is obviously higher than the stable ones(27.78%) and people without plaque(29.17%).3 The average contents of E- selectin and MMP1 are lower in HCMV-PP65 negtive ones than positive patients,the contents are 2.33ng/ml to 2.76 ng/ml and 1.50 ng/ml to 2.33 ng/ml.4 In the positive cases, in the patients without plaque the contents of E -selectin is 0.59 ng/ml, MMP1 is 0.38 ng/ml. in the atherosclerotic plaque patients the contents of E-selectin is 2.70 ng/ml, MMP1 is 1.70 ng/ml.All of the results are higher than the ones without plaque.5 Compare the contents of E-selectin and MMP1 between the group with instable plaques and stable ones:In the group with instable plaques, the contents of E-selectin is 1.82 ng/ml, MMP1 is 0.68 ng/ml,and in the people with stable plaques, E-selectin is 2.64 ng/ml, MMP1 is 1.80 ng/ml, all of the results are higher in instable ones than the stable,the contents of MMP1 has statistics error,but the contents of E- selectin has no statistics errorConclusions:There is obviously association between activated infection of HCMV and ischemic cerebrovascular disease,compare with the health people the patients with ischemic cerebrovascular disease have a high positive rate of HCMV-PP65. Patients with instable plaques have high positive rate of HCMV-PP65 than patients with stability plaques and without plaques. HCMV can increase the expression of E-selectin and MMP1,and it is higher than HCMV-PP65 negtive ones.
