The Expression And Prognosis Of BCL-10 And C-MYC In Ocular Adnexal MALT Lymphoma

Objective:1.We review 22 cases of ocular adnexal MALT lymphoma according to their clinicopathologic features,immunophenotype,with the purpose to identify unfavorable prognostic factors.2.Detect the expression of BCL-10 and C-MYC in ocular adnexal MALT lymphoma and explore the prognostic significance.Methods:1.We retrospectively analyzed the clinical characteristics of 22 ocular adnexal M ALT lymphoma cases who were diagnosed in Tianjin Eye Hospital from 2001 December to 2004 February.2.The morphologic spectrum,the expression of BCL-10,C-MYC through immunohistochemistry staining in specimens available and the relationship between the protein expression status and patients'clinical and prognositic characteristics were investigated.3.All patients were followed up and divided into two groups according to the paterns of BCL-10 and C-MYC expression for clinicopathologic features study and survival analysis.Results:1.The group's clinical material indicates that Stage of disease wasⅠfor 15 patients,Ⅱfor 5 patients,Ⅲfor 1 patient,Ⅳfor 1 patient.In the tear gland for 6 patients,the conjunctiva for 2 patients,others occur in the orbit.The morphology of the tumor cells was transform to diffused large B cell for 7 patients,centrocyte-like cells for 18 patients,monocytoid cells for 4 patients.4 patients were dead of tumor recrudescence.2.In 20 of 22(90.9%)ocular adnexal MALT lymphoma cases nuclear and/or cytoplasmic BCL-10 expressions were seen,in which 11 of 22(50%)ocular adnexal MALT lymphoma cases exhibited BCL-10 expression in both nucleus and cytoplasm, whereas in other 9 of 22(40.9%)cases there was only cytoplasmic BCL-10 expression.In 8 of 22(36.4%)ocular adnexal MALT lymphoma cases nuclear and/or cytoplasmic BCL-10 expressions were seen.The expression of the BCL-10 and C-MYC don't have the relevance.Conclusion1.Histological observation revealed that diffusing infiltration of tumor cells was found mainly in mucosa,and that tumor cells showed characteristics of center-like cells and monocytoid cells.Follicles could be found in the genesis center at the inner edge of tumor in part of the cases.2.the clinical stage of the ocular adnexal MALT lymphoma is correlated with the prognosis.3.BCL-10 protein may express either both in nucleus and cytoplasm or only in cytoplasm,of which the former is more important in the pathogenesis of ocular adnexal MALT lymphoma.BCL-10 nuclear expression is correlated with the clinical stage in ocular adnexal MALT lymphoma(P＜0.05).4.C-MYC nuclear expression is correlated with the clinical stage in ocular adnexal MALT lymphoma(P＜0.05).Examination of the C-MYC protein is reliable appraisal in the biology behavior,and has the good guidance in the prognosis.
