Objective: To explore the value of Cyclin E and P27 protein in predicting malignant transformation of hydatidiform mole by analyzing the variability of their expression between hydatidiform mole with and without malignant transformation and reaserching the relationship of them with malignant high risk factors and different hyperplasia of trophoblastic cell.Methods: The expression of Cyclin E and P27 protein were detected by immunohistochemistry (PV-9000) in 15 cases of normal early pregnancy villis and 30 cases of hydatidiform moles(including 6 cases of malignant transformed moles of which 5 cases changed to invasive hydatidiform mole within 1 years and 1 case changed to chorionic carcinoma 3 years later).Results:1. The expression of Cyclin E and P27 protein were detected in both normal early pregnancy villi and hydatidiform mole.2. The expression of Cyclin E protein was significantly higher in complete hydatidiform mole than that in normal early pregnancy villi and partial hydatidiform mole(P < 0.05).There had no difference between normal early pregnancy villi and partial hydatidiform mole (P >0.05) .There had no significant difference between hydatidiform mole with and without maliganant transformation (P>0.05) .3. The expression of Cyclin E protein was significantly higher in hydatidiform mole with malignant high risk factors (such as uterus > gestation weeks, HCG>100000U/L, ovary lutein cyst of ovary > 6cm) than that without malignant high risk factors (P<0.05).There had no significantly difference between the group of age (P>0.05) .4. The expression of P27 protein was significantly lower in complete hydatidiform mole than that in normal early pregnancy villi and partial hydatidiform mole(P < 0.05).There had no significant difference between normal early pregnancy villi and partial hydatidiform mole (P >0.05) .There had significant difference between hydatidiform mole with and without maliganant transformation (P<0.05) .5. The expression of P27 protein was significantly lower in hydatidiform mole with malignant high risk factors (such as uterus > gestation weeks, HCG >100000U/L, ovary lutein cyst of ovary >6cm) than that without malignant high risk factors (P<0.05).There had no significant difference between the group of age (P>0.05) .6. There had an inverse relationship between the protein expression of Cyclin E and p27 in hydatidiform mole (r=-0.874, P<0.05) .7. When the hyperplasia degree of trophoblastic cell rasing, the expression of P27 descending and the expression of Cyclin E ascending. Conclusion:1. When the expression of P27 protein is poor (-or +) in hydatidiform mole,it may be malignant transforme,and when there have more than one malignant high risk factors,the rate of malignant transformation would be increase. It can be one index for predicting malignant transformation of hydatidiform mole.2. Descending tendency of P27 and ascending tendency of Cyclin E expression are found between normal early pregnancy villis and hydatidiform moles. When the expression of Cyclin E protein is strong positive and P27 is negative exsiting in hydatidiform mole together,it may be malignant transforme more possibly. It may be more significant to predict malignant transformation of hydatidiform mole when detecting the expression of Cyclin E and P27 protein together.3. There has a closely relationship of the different hyperplasia of trophoblastic cell with the expression of Cyclin E and P27 protein.
