The Clinical Effect Analysis Of The Low Molecular Weight Heparin In The Treatment Of Intrahepatic Cholestasis Of Pregnancy

Objects:To analysis the clinical effect of the low molecular weight heparin in the treatment of Intrahepatic cholestasis of pregnancy.Methods:We retrospectively analysed 16 intrahepatic cholestasis of pregnancy (ICP)patients randomly who were treated with additional use of the low molecular weight heparin(LMWH)as the LMWH group,and at the same period,randomly chose 23 ICP patients who were treated with the conventional drugs as the non-LMWH group.The course of treatment was according to the patients' situations.The blood samples in the two groups were extracted both before and after the treatment,and measured the ALT,AST,TBA with biochemistric analysator.The pruritus symptoms relief days during the treatment was recorded.We recorded the situations of the delivery,including the fetal birth weight,the fetal Apgar's score,the contamination ratio of amniotic fluid and the quantity of hemorrhage during the labor.Results:(1)The value of ALT in the LMWH group and the non-LMWH group were 141.96±91.34U/L and 159.42±141.47U/L.The result manifested that the descent of ALT in the LMWH group (67.43±33.20U/LU/L)was lower than that in the non-LMWH group (119.50±115.67U/L),and it had statistic difference(P＜0.05).The value of AST in the LMWH group and the non-LMWH group were 111.99±104.39U/L and 118.21±106.87U/L.The result manifested that the descent of AST in the LMWH group(55.94±41.99U/L)was lower than that in the non-LMWH group(80.19±74.82U/L),and it had statistic difference(P＜0.05).(2)The value of TBA in the LMWH group and the non-LMWH group were 50.27±45.49μmol/L and 42.95±36.37μmol/L.The result manifested that the descent of TBA in the LMWH group (18.01±15.67μmol/L)was lower than that in the non-LMWH group (24.99±14.10μmol/L),and it had statistic difference(P＜0.05). (3)The pruritus symptom relief days in the LMWH group (4.06±2.49d)was shorter than the non-LMWH group(5.45±1.50d),there was statistic difference(P＜0.05).(4)The fetal birth weight(FBW)in the LMWH group and the non-LMWH group were 2782.13±572.08g and 2838.00±451.87g,there was no statistic difference(P＞0.05).(5)The fetal Apgar's score at one minute between 0～7 in the LMWH group(1/16)was as the same level as in the non-LMWH group (3/20),and showed the asphyxial ratio between the two group had no statistic difference(P＞0.05).But at 5 minute,both groups' scores were between 8～10.(6)In our study,the contaminate ratio of amniotic fluid in the ICP patients was 44.44%.There was statistic difference between the LMWH group(25.00%)and non-LMWH group(60.00%)(P＜0.05).(7)The quantities of hemorrhage in the LMWH group and in the non-LMWH group were 216.88±65.50ml and 207.50±49.40ml.There was no statistic difference(P＞0.05).Conclusions:(1)The treatment of Intrahepatic cholestasis of pregnancy(ICP) which is added with the low molecular weight heparin(LMWH)has better effect compared with the conventional treatment.(2)LMWH is a kind of safe and effective drug in short-time application,and it could be used as a kind of routine drugs in the treatment of ICP.
