The Surgical Treatment Of Chondrosarcomas: A Clinical Outcome Study

objective:To evaluate the outcome of surgical operation and modus operandi for chondrosarcomas in the appendicular skeleton and the axial skeleton.Methods:A retrospective analysis was conducted on clinical data from 47 patients with chondrosarcoma undergoing surgical operations from 1993 to 2007.Tumor location:25 cases of limbs long bone(three cases of the proximal humerus,one case of the middle of the humerus; eleven cases of the proximal femur,five cases of the middle of the femur;three cases of the proximal tibia,one case of the middle of the tibia,one case of the proximal tibula);axial skeleton 22 cases,including 15 cases of the pelvis(five cases of the ilium,three cases of the acetabulum, two cases of the sacrum,two cases of the ischium,three cases of the pubis),vertebral column 7 cases(three cases of the lumbar vertebrae,two cases of the thoracic vertebrae,two cases of the sacral vertebra).Among them,29 patients received bone tumor resection,14 patients received bone tumor resection and prosthetic replacement,and 4 patients underwent amputation.A patients had three times bone tumor resections;postoperative adjuvant chemoradiotheray were performed in 6 patients and all patients did not have preopeative adjuvant chemoradiotheray.43 patients were followed up for 4 to 124 months.The mean follow-up period was 51.36 monthsResults:In the appendicular skeleton group,local recurrence rate was 27.27%,the proportion of patients with 3-year overall survival was 95.45%;In the axial skeleton group,local recurrence rate was 61.90%,the proportion of patients with 3-year overall survival was90.47%. There was significant difference between the two groups in local recurrence rate(P＜0.05).In the bone tumor resection and bone graft group,local recurrence rate was 57.14%;in the bone tumor resection and prosthetic replacement group,local recurrence rate was 16.67%,there was significant difference between the two groups in local recurrence rate(P＜0.05).Conclusion:.(1)Postoperative local recurrence rate in the axial skeleton compare the appendicular skeleton set in high.(2) Postoperative local recurrence rate in the bone tumor resection and bone graft group compare the bone tumor resection and prosthetic replacement group set in high.
