Objective: (1)To study the detail anatomy and improved surgical approach for fractures of the body, collum and cavitas glenoidalis of the scapula, and to apply this anatomical evidence in clinical practice. (2)Also to analyse the clinical data of scapula fractures diagnosis and treatment.Method: (1) The regional anatomy of important nerves and blood vessels in the spatium intermusculare between infraspinatus and teres minor,teres minor and teres major,and their relationship to the improved surgical technique were studied using the 20 sides of 10 cadevers.(2)In addition an analysis of 29 cases of farctures of the body, collum and cavitas glenoidalis of the scapula that were treated ,at The Guang Xi Medical University First Affiliated Hospital, with the improved surgical technique was also done. The appraise therapeutic result is basis on Hardergger's standard of functional assessment.Results: (1) The improved surgical approach does not utilise the triangular and quadrangular spaces. As such it only transect the branches of the suprascapular and circumflex scapular artery . It does not endanger any major nerve or blood vessels(axillary nerve,circumflex scapulae and circumflex humeri posteriot artery). This approach provides good access and clear vision for body, collum and cavitas glenoidalis fracture of the scapula. (2) The application of this approach in the 29 cases of fracture scapula treated yield excellent results 86.2% of the cases. There was no post operative complication.Conclusion: The improved surgical approach for fractures of the scapula has many advantages including; a small incision, minimal surgical trauma, a lower risk of major blood vessel or nerve injury, good access to the fracture facilitating easy fixation ,and a shorter post operative recovery duration. This approach is certainly worth popularising.
