Metabolism syndrome(MS)is a kind of metabolic disorder syndrome that multipe cardiovascular disease risk factors such as obesitas,hypertension,abnormal sugar metabolism and dyslipidemia concentrate on one body.Insulin resistance(IR)was ever generally considered as its physiopathologic base.When the comprehansion to MS keeps advancing,it is more and more clear that dyslipidemia caused by obesitas plays an important role in formation of MS.It has been certified that in every symptom grouping(components)of MS obesitas is the important abnormal metabolism index. Abnormal sugar and lipid metabolism and hypertension maybe its result.A lot of domestic and foreign epidemiological investigations have shown that the morbidity is affected by kinds of enviromental and genitic factors,vary wildly in different areas, races and social economic status.Although there are may domestic and foreign studies, Xiaoshan Distric Hangzhou city which belongs to southeast littoral developed area has been highly urbanized,especially recent years,foreign joint ventures and sole corporations increase greatly and Xiaoshan private businesses develop rapidly.So floating population residing in Xiaoshan over one month attain 800,400,permanent population is 1198,000.That added new reconstituents in local culture,life style and dietary composition.The traditional life style has been broken into a multicomponent system.The extant articles cannot exactly present the character of local district.This study is made to clarify the morbidity and distribution character of metabolism abnormality and MS in local district,to analyze risk factors and high risk population of MS in the district,to provide the useful information to prevention,intervention and therapy.Objectives:1.To clarify the morbidity and distribution character of MS and its every component among elderly people in Xiaoshan District.2.To discuss the contribution of sex,age and every component of MS.3.To study the effect and character of obesitas in metabolism abnormality and MS in local district.4.To determine the risk factors and high risk population of MS in local district. To provide useful information to prevention,intervention and therapyMethods:We adopted cross-sectional study,3917 cases were picked up as samples to be measured height,weight,blood pressure,fasting plasma sugar and lipid among the elderly people who came to our hospital for health examination from January to June in 2007.According to the diagnostic standard of MS setted by Chinese Diabetes Society in 2004,weight index being as the evaluation criterion of overweitght and obesitas,we applied SPSS 14.0 for windows software to data processing and statistical analysis, while p≤0.05,the result was considered possessing statistical significance.Forward stepwise regression analysis of Binary logistic was applied to analyze the correlation between MS and weight index,age,every component of it and their effects.Results:1.The standardized MS morbidity among elderly people in Xiaoshan is 21.81%. female is 23.39%,male is 20.37%.Sex contrast had statistical difference(p<0.01). middle-aged men and old women are high incidence population in the local district.2.The mobidity of each abnormal metabolism component among elderly people in local district is below:overweigh/obesitas 52.36%,high blood sugar 25.56%, hypertension 50.37%,dyslipidemia 52.41%.Female morbidity is higher than male in the most abnormal metabolism components of MS group.The most concentrating mode is overweight/obesitas+hypertension+dyslipidemia in every abnormal metabolism component. 3.When overweight/obesitas index delaminated,following the increase of weight index,the male and female MS morbidity presented obvious ascending tendency (p<0.01).The same tendency also occured in every abnormal metabolism component (p<0.01).Each cardiovascular risk factor MS involved in concentrated to a peak on obesitas.In the 927 cases of MS diagnosed according to the diagnostic standard setted by Chinese Diabetes Society,overweight/obesitas group ocupied 87.81%,thin and normal figure group only 0.26%and 11.97%.4.Forward stepwise regression analysis of Binary logistic show there were 5 factors of female being selected in turn,weight value,blood sugar,triglyceride,age and high density lipoprotein.Population grouping capability of the equation was 83.7%after 5 factors being selected.P<0.01 through x~2 test.there were 4 factors of male being selected in turn,triglyceride,weight value,blood sugar and age.Population grouping capability of the equation was 84.8%after 4 factors being selected.P<0.01 through x~2 test.The result show weight value had the rather close correlation with MS in both male and female which proved obesitas play a more important role than the other components in the formation of MS.Conclusions:1.There is rather high morbidity of MS and every abnormal metabolism components among elderly people in Xiaoshan,and higher than Beijing-like large cities and Foshan-like areas with the similar economical level.2.Obesitas plays an important role in the formation of MS and abnormal metabolism.3.There is area difference between concentrating modes of each abnormal metabolism component.The work of prevention and treatment for Ms should take obesitas as target,at the same time,combining local district characters.4.Middle-aged male and old female who are suffering from obesitas should be the intervented,prevented and controled population in the local district.
