Study On The Governance Structure Of New Rural Cooperative Medical System In China

To establish a new type of rural cooperative medical system is an important measure to implement the scientific concept of development, ensure stability and harmony in rural areas and build a well-off society comprehensively. In order to make rural cooperative medical system be a stable system, the cooperation among stakeholders is needed. Because economic people's nature is opportunism, the cooperation among people is cooperative games under certain conditions. The main stakeholders of rural cooperative medical system include governments, farmers, and medical constitutions. Driving by the utility maximization, they not only cooperate but also constraint one another during the operation of rural cooperative system and determine the system's existence and disappointment. However, because of the profits games among different governments, different farmers, different medical constitutions, the new rural cooperative system is complicated and special.This paper inspects the development process of Chinese rural cooperative medical system from the perspective of history transformation, sums up the different connotations of cooperation, arrangement of the traditional rural cooperative medical system in China and the factors of its great success. This is followed by summarizing the system structure and efficiency characteristics of Shanghai's new rural cooperative medical system, finding the problems which exist on that kind of governance structure. After that, this paper studies the game among governments, farmers, medical institutions and the profit game among different governments, different farmers, and different medical institutions. Governance structure is key to solve the issues of the supply-induced demand, adverse selection and moral hazard of cooperative medical system. Finally, this paper puts forward governance principles, the framework of the objectives and recommendations to establish the governance structure of new rural cooperative medical system. This paper specifically includes the following six parts:Chapter 1: Introduction. This chapter focuses on topics background, the purpose and significance of research, developments at home and abroad, sums up the research ideas and methods and the main innovation of this paper.Chapter 2: Basic theory of governance and its applications. The first is the definition of governance, governance characteristics, the theory's application in corporate governance and the village governance, then is the analytical framework of public service governance structure, and finally the application of the governance in the rural cooperative medical system.Chapter 3: Historical transformation of rural cooperative medical system in China. This chapter focuses on the development process and the governance structural conditions of traditional rural cooperative medical system, the transition of rural cooperative medical system and the new rural cooperative medical system. This chapter makes the comparative analysis from the perspective of the cooperation areas, security objectives, institutional environment and the role of goverments.Chapter 4: Existing problems of new-type rural cooperative medical system governance structure. This chapter analyzes in Shanghai's rural cooperative medical system governance structure as an example, reviews the successful experiences of Shanghai's rural cooperative medical system governance structure.
