Objective:Trauma including burn(scald)intestinal injury in systemic inflammatory response syndrome(SIRS)and multiple organ dysfunction integrated syndrome (MODS)and development play an important role.To study the mechanism and protective of intestine damage after trauma,is very important.In this experiment we investigated activity of blood DAO and changes of gene expression of TNFα,IL-10 and NF-κB by setting up the model of severe scald injury in rats,and further research the protective effects of GIK(glucose-insulin-potassium chloride)on intestinal injury with severely scalded rat.To discussion the function of inflammatory response and GIK in intestine injury with trauma,the result can be provided the experimental basis for its clinical application.Methods:Male or female Sprague-Dawley(SD)rats(200-250g)were randomly allocated into three groups(n=6,normal control group,n=18 for scalded group and GIK intervention group):control group;scald group;GIK intervention group.Base upon the Walker's and Mason's model,we set up the scalds model,these rats were subjected to a30%total body surface area(TBSA)full thickness scald.1.Normal control group:we didn't scald rats and didn't give the medicine.2.Scald group:Base upon the Walker's and Mason's model,we set up the scalds model.After setting up the model,we give 0.9%Nacl by intraperitoneal injection,dose as 20ml/Kg/8h.3.GIK intervention group:we give GIK by intraperitoneal injection,dose as 20ml/Kg/8h.After setting up the model. The rats were sacrificed by sudden death due to heart failure after scald at 1.3.5 days respectively.The levels of DAO,glucose and insulin in blood serum of each group were detected.The activity of NF-κB in intestine tissue was measured by immunohistochemistry and intra-intestine TNF-αmRNA,NF-κB and IL-10 gene expression in intestine were studied by semi-quantitantive RT-PCR.Results:Compared with normal control group,the level of biochemical indicator of scald group and GIK intervention group is increased significantly(P<0.01or P<0.05), otherwise,blood DAO,glucose,insulin,was ameliorated in scald with used GIK group.it proved that GIK can improve the metabolic disorder of organism.Compared with normal control group,observation of histological sections of intestine stained with HE under light microscopeshows shows that gut edema, enterocyte necrosis and fall off,enteric epithelium separates with Intestinal lamina propria is not integrity in intestine of scald group(P<0.05),but it's not so seriously in scald with use GIK group(P<0.05),which suggested GIK can protect intestine against intestine injury of scald.The level of NF-κB,TNF-αand IL-10 mRNAs were increase obvious in scald group contrast with normal group(P<0.01).But expression of these mRNAs was decrease significantly in GIK group contrast with scald group(P<0.01).Conclusion:1.Severely scalded rats represent the obvious intestinal injury and high activity of blood DAO.2.Severely scalded rat intestinal injury was associated with hyperglycemia.3.Severely scalded rat intestinal tissue TNFαand IL-10 gene expression unbalances and gene overexpression of NF-κB in intestine injury,play an important role.4.GIK treatment by 1)lowering blood glucose severely scalded rats.2)lower intestine of NF-κB and TNF gene overexpression,3)up-regulation of IL-10 gene expression,control as a serious intestinal injury trauma of the important measures.
