Objective: Gouty arthritis due to crystals of monosodium urate deposition in the tissues around joints caused by acute inflammatory response. The main performance of joint is red, swelling, heat, pain and dysfunction, repeated attract. Now many scholars in the domestic Chinese herbs gout treatment has been got some achievements, but throughout literature, and there is not the report made significant treatment on a traditional Chinese medicine. According to Coderre on this experiment urate sodium crystal-induced rat paw edema model for improving slightly, by observing traditional Chinese medicine and western medicine, that is colchicine for Tongfengyin 1 on gouty arthritis in rats limb pressure points, gait, foot swelling, WBC, neutrophil, uric acid, IL-1β, IL-6 in rat serum gouty arthritis of expression, as well as the structure of the pathological effects of Tongfengyin 1 in the treatment of acute gouty arthritis would be the pharmacological effects to Chinese medicine treatment of acute gouty arthritis provide experimental basis.Methods:(1) Selection of 48 Wistar rats were randomly divided into four groups, each with 12, were gaps in the normal control group (which was abbreviated to"N group"), the colchicine positive control group (which was abbreviated to"C group"), the Chinese medicine treatment of Tongfengyin group (which was abbreviated to"T group"), the model (which was abbreviated to"M group"). After the rats were fed adaptively for a week, the experiment was done. The intra-articular of right ankle including C, T, M group were injected with 0.2ml urate sodium suspension (which was abbreviated to"MSU"), and the same time the intra-articular of right ankle of N group was injected with 0.2 ml saline. In the 24 hours after making model on rats, according to people with the experimental dosage of animals dosage the conversion formula, the dosage was calculated in rats, T group was dosed with 13.5 g / kg ? d, C group was dosed with 0.135 mg / kg ? d, all drug of gavage was divided into twice, the total of gavage for one day. M group and N group were dosed with saline as control at the same time. (2) In the 24 hours after making model and the 24 hours after treatment on rats, observe the general performance and the arthritis of joint, and make score for limb gait and pressure points; before making model, in the 24 hours after making model and the 24 hours after treatment on rats, the right ankle perimeter were measured, and calculate changes in the ankle circumference of the value. In the 24 hours after treatment, decapitated the rats by counting interleukin count-law, the blood-Tu, dyeing, do interleukin artificial classification, centrifugal separation serum, spare, measured UA level of serum by enzyme assay, measured IL-1β, IL-6 levels of serum by radioimmunoassay method.(3) After the rats were killed, the tissues of right ankle joint were acquired, with 10% formalin-fixed, paraffin-embedded, to hematoxylin - Yihong (which was abbreviated to HE) staining, to observe by light microscope inflammatory leukocyte infiltration, capillary cavity size and was made photos by HPIAS-1000 High-resolution color pathological analysis system.(4) Use SPSS11.5 for Windows statistical software to analyze the above-mentioned data, and use Nemenyi to analyze rating information .Results:1 the general behavior of all the ratsDuring the process of making model, the rats of N group was luster, the eyes and activity were nimble, and having sensitive reflection; the rats of C group, T group and M group which were induced inflammation were swelling , unsteady gait, suffering from foot upward, and screaming. After the treatment, the rats of C group and T group were improved in their mental state, reduced the swelling of feet, reduced the limp.2 Effect of Tongfengyin on the gouty arthritis in rats standing and walking limb pressure integral value, and the right ankle perimeter2.1 After making model, the limb pressure integral value in rats when standing and walkingThe limb pressure integral value in rats of C group, T group, M group rats limb when standing and walking were significantly higher than that of N group (P<0.01).2.2 After making model the changes of right ankle perimeterThe changes of right ankle perimeter of C group, T group, and M group were significantly increased compared with N group (P<0.01).2.3 After treatment, the limb pressure integral value in rats when standing and walkingThe limb pressure integral value in rats when standing and walking of C group, M group still significantly increased compared with N group (P<0.01). That of T group is decreased compared with M group, with statistical significance (P<0.05).2.4 After treatement, the changes of right ankle perimeterThe changes of right ankle perimeter of T group were statistically significant compared with M group, N group (P<0.05).That of C group and T groups had no statistical significance (P>0.05).That of C group and M group had no statistical significance (P>0.05).3 Effect of Tongfengyin on Interleukin and the percentage of Neutrophil in InterleukinInterleukin of M group was significantly higher than that of C group, T group, N group (P<0.01), and between the three groups had no significant difference (P>0.05).The percentage of Neutrophil in Interleukin of C group and T group were decreased compared with M group (P<0.01). That of T group has no significant difference compared with N group (P>0.05).4 Effect of Tongfengyin on IL-1β, IL-6The levels of IL-1βin plasma of C group, T group have no significant difference compared with the N group (P>0.05), that of group M were significantly increased compared with N group, T group (P<0.01).That of C group has no significant difference compared with T group (P>0.05).The levels of IL-6 in plasma of C group, T group, M group were significantly increased compared with N group (P<0.01), that of M group was significantly increased compared with C group, T group (P<0.05). That of C group was not significant compared with T group(P>0.05).5 Effect of Tongfengyin on uric acid in plasmaThe levels of uric acid of T group in plasma increased compared with N group (P<0.05),and that of N, C, M group among the three groups had no significant difference (P>0.05).6 Effect of Tongfengyin on the pathological form of ratThe synovial lining layer of N group was composed by layer 1 to 2 synovial cells, and some did not continued, and synovial lower was fat cells, a small amount of blood vessel cells and fibroblasts. The synovial lining layer of M group was significantly thickened, shaped by"fence"array or irregular arrangement. The synovial tissue was villous or finger-like mast, the lining cells of the fat cells was decreased or even disappeared, and a large number of fibroblasts and collagen fibers were appeared. The injury site and the surrounding was stacked the inflammatory cells, polymorphonuclear leukocytes, monocytes, and foreign body giant cells, etc. There were multiple angiogenesis, and vascular cavity expanded and was filled with blood obviously. The aboved-mentioned appearance of C group and T group was lower than that of M group.Conclusion:Tongfengyin has a good inhibited role on acute gouty arthritis. Its mechanism may be through inhibiting neutrophil-mediated inflammation, the main inhibiting pro- inflammatory cytokine IL-1β, followed by two inhibition of inflammatory cytokines IL-6 to achieve anti-inflammatory effect. Tongfengyin can reduce serum uric acid levels are pending further investigation.
