OBJECTIVE:Although chronic prostatitis is a common disease in urology,our understanding of the etiology,diagnosis and treatment of prostatitis has not advanced with that of other prostate diseases.Our study try to measure the cytokines IL-2,IL-10 and TNF-α,which are important mediator of inflammation,in EPS of health men,men with CBP,men withⅢa CPPS andⅢb CPPS,to discuss the possible mechanism of prostatitis and understand the roles of cytokines in the diagnosis and treatments of prostatitis.METHODS:From September 2007 to February 2008,fitly-eight men seen in the Third XiangYa Hospital were enrolled to participate in the research.All the patients had been suffering from the chronic prostatitis syndrome.Patients were classified into three categories according to the NIH diagnosis criteria for chronic prostatitis:chronic bacteria prostatitis(CBP)(n=11),inflammatory CPPS(n=26), noninflammatory CPPS(n=21).And we chose 12 normal people as a contract.Cytokine levels were measured in EPS by two-antibody enzyme-linked immunosorbent assay.All the data were analyzed by the statistical package SPSS for windows 12.0.RESULTS:The mean age of control group,CBP,Ⅲa CPPS andⅢb CPPS was 27.4,32.5,29.0 and 29.7 respectively,and there were no statistically significant differences between them.The NIH-CPSI score of: control group,CBP,Ⅲa CPPS andⅢb CPPS was:0,18.3±4.4,17.6±3.4, 17.1±3.0,there were no statistically differences among all CP groups.The WBC counts of control group,CBP,Ⅲa CPPS andⅢb CPPS was 1.5, 31.2,25.1 and 4.4 respectively.There were no statistically significant differences between CBP group andⅢa CPPS group,and the difference between control group andⅢb CPPS is also insignificant.The level of TNF-αin control group,CBP group,Ⅲa CPPS andⅢb CPPS group was 17.8±9.2pg/ml,91.5±34.8 pg/ml,74.9±35.1pg/ml,23.9±12.9 pg/ml, groups for CBP andⅢa CPPS have higher mean levels of TNF-αthan control group andⅢb CPPS group,and the differences between them was statistically significant.The mean level of TNF-αin control group andⅢb CPPS group has no statistically significant differences.The level of IL-10 in each group were:the control group 234.9±56.6pg/ml,the CBP group 392.7±62.4pg/ml,theⅢa CPPS group 364.1±77.1pg/ml,theⅢb CPPS group 300.4±80.2pg/ml,groups for CBP andⅢa CPPS have higher mean levels of IL-10 than the control group andⅢb CPPS group, and the differences between them are statistically significant.The level of IL-2 in each group were:the control group 311.4±67.5pg/ml,the CBP group 247.7±96.1pg/ml,theⅢa CPPS group 278.2±77.6pg/ml,theⅢb CPPS group 299.7±78.6pg/ml,the level of IL-2 in the CBP group is lower than the control group,and the difference between them is statistically significant.There were no differences between the levels of other groups.There is a low correlation between IL-10 and TNF-α(r=0.26,P(0.05),IL-10 and IL-2(r=-0.286,P <0.05).The levels of TNF-αin CP patients has a correlation with the WBC counts of EPS(r=-0.772, P <0.01).The levels of IL-10 in EPS of patients with CP has a positive correlation with the pain scores(r=-0.549,P <0.01).CONCLUSION:1.The level of pro-inflammatory cytokine TNF-αand anti-inflammatory cytokine IL-10 in CBP group andⅢa CPPS group is higher than that of the control group andⅢb CPPS group.The level of trans-inflammatory cytokines IL-2 is lower in CBP group than that of the control group.It shows that these three cytokines probably participate the pathogenesis of CP and immune factors may play an important role in CP.2.There are no statistically significant differences of the levels of IL-10 and TNF-αbetween CBP group andⅢa CPPS group,we speculate that the mechanism of CBP andⅢa CPPS may be similar.3.There are no statistically significant differences of the level of IL-10 and TNF-αin the control group andⅢb CPPS group,factors outside the prostate may play an important role in pathogenesis ofⅢb CPPS,for example,the disorder of pelvic nerves function or psychological factor. 4.The level of IL-10 and TNF-αinⅢa CPPS group is higher thanⅢb CPPS group,and the difference is statistically significant,IL-10 and TNF-αmay become an indicator for the diagnosis of CP.5.There is a correlation between the WBC counts and the level of TNF-αin EPS of patients with CP(r=-0.772,P <0.01),which indicate that TNF-αmay play an important role in the inflammation of prostate.6.The level of IL-10 in EPS of CP patients has a positive correlation with the pain scores(r=-0.549,P <0.01).IL-10 may cause the syndrome of pain, and the level of IL-10 may be an indicator to evaluate the effect of treatments.
