Preliminary Study Of Relativity Between Visfatin And Gestational Diabetes Mellitus, Gestational Obesity, Fetal Macrosomia

Chapter oneThe visfatin of gestational diabetes mellitus, gestational obesity and insulin resistanceObjective: To explore the relationship between visfatin of gestational diabetes mellitus,gestational obesity and insuline resistance.Methods: The body weight of progestation and prior to delivery;fasting plasma glucose(FPG),fasting insuline, blood fat(cholesterol and triglyceride) was measured in 45 primiparae with singleton birth, which include 15,15 gravida complicating obesity and 15 control group in the sixth people hospital of shanghai from 2006 to 2007. Then calculate their IR-index, the visfatin level of cord and maternal serum and determine the visfatin level of their placenta.Result:(1) There is visfatin mRNA expression in placenta.(2) Contrasting the control group, the maternal weight gain, fasting plasma glucose ,fasting plasma insulin, triglyceride, the IR-index and the BMI of prior to delivery are increased significant in gestation diabetes mellitus group and gestational obesity group.(3) The maternal visfatin level are significantly increased in gestation diabetes mellitus group and gestational obesity group than in the control group.(4) The visfatin level of the placenta are higher in the gestation diabetes mellitus group and gestational obesity group than in the control group.Conclusion: The visfatin level of maternal and placenta are increased in gestation diabetes mellitus and gestational obesity.The increasing was relatived with insulin resistance.Charpter twoThe visfatin of fetal macrosomia and insulin resistanceObjective: To explore the relationship between visfatin of fetal macrosomia and insuline resistance.Methods: The body weight of progestation and prior to delivery; fasting plasma glucose(FPG),fasting insuline, blood fat(cholesterol and triglyceride) was measured in 53primiparae with singleton birth, which birth include 23 fetal macrosoma and 30 normal weight fetals in the sixth people hospital of shanghai from 2006 to 2007. Then calculate their IR-index and the visfatin level of cord and maternal serum .Result:(1)Contrasting the control group, the maternal weight gain,fasting plasma glucose ,fasting plasma insulin and triglyceride, IR-index, the BMI of prior to delivery and the BMI of progestation in fetal macrosoma group are increased significant.(2)The positive correlation was found between the maternal weight gain,fasting plasma triglyceride ,fasting plasma glucose,the BMI of prior to delivery,the BMI of progestation and the fetal weight.(3)The visfatin level of maternal is significantly higher than the control group.(4) The visfatin level of maternal serum is the independent correlation factor of fetal weight.Conclusion: The incidence of fetal macrosomia is association with the insulin resistance and the elevated maternal visfatin concentration.
