This thesis preliminarily explores the related assessment and index of examination of the treatment of cervical spondylosis of arterial type (CSA), and the effect and mechanism of the treatment by electroacupuncture combined with Danggui injection into points mainly from the following aspects: the literature review, the theoretic discussion, the Clinical Study and the statistical analysis.Objective : to observe the effect of the treatment of CSA by electroacupuncture combined with Danggui injection into points, to explore the mechanism of the therapy, to provide a kind of simpler and more effective method for treating the disease.Methods: According to the diagnostic criteria of CSA from the diagnostic criteria and typing standard drafted by the second national special conference summary of cervical spondylosis (1992. Qingdao), 180 in-patients with CSA were randomly divided into the treatment group and the control group, who were hospitalized in the department of Acupuncture-Moxibustion and the department of Neurology of Shiyan Taihe Hospital , and the department of Rehabilitation of Zhongnan Hospital of Wuhan University from July 2006 to October 2007.Every group had 90 cases.In the treatment group,electro-acupuncture combined with Danggui injection into acupoints was used.The acupoints stimulated by electroacupuncture are three pairs of Jiaji points of cervical vertebrae,which took he vertebral body of the lesion as the center and were selected. Needling:The position of the patients was prone-torso sitting. After routine disinfection,the acupoints were needled 0.8-1.0 cun perpendicularly using the mild reinforcing and reducing of the twirling and rotating until the needle sensation radiated to the neck and shoulder. The electroacupuncture instrument( 6805 )was used with the sparse waves (2-10HZ) and the current intensity within a range which was tolerable.The needles were retained for 30 minutes, once a day. Then, Danggui injection was injected into the positive sensitive points of the lesion location of cervical vertebrae.5ml was injected into every point, once a day.The control group was treated once a day with the intravenous drip of vasodilator and antispasmodic,such as Tetramethylpyrazine and 654-2. The treatment lasted two weeks. The symptom and function scoring table of CSA was observed.The transcranial Doppler (TCD) were adopted to detect the mean velocities(Vm) of vertebral artery(VA) and basilar artery ( BA ) before and after the treatment.Hemorrheology was measured to study the whole blood viscosity(the tallcut, midcut, lowcut)and plasma viscosity. The data was analyzed by the statistical software SPSS (14.0) .Results: (1)The total effective rate was 94.4% in the treatment group and 65.6% in the control group with a significant difference between the two groups(p<0.01). The therapatic effect of the treatment group was superior to that of the control group.(2)Before the treatment, the symptom and function scoring was comparable without a significant difference between the two groups(p > 0.05).After the treatment, the symptom and function scoring of the treatment group was more than that of the control group with a significant difference between the two groups(p <0.01).(3) Before the treatment, the Vm of VA was comparable without a significant difference between the two groups(p>0.05). After the treatment, the Vm of VA of the two groups increased.The Vm of VA of the treatment group was much higher than that of the control group with a significant difference between the two groups(p <0.01)( 4) Before the treatment, the Vm of BA was comparable without a significant difference between the two groups(p>0.05).After the treatment, the Vm of BA of the two groups increased.The Vm of BA of the treatment group was much higher than that of the control group with a significant difference between the two groups(p<0.01).(5)Before the treatment,the tallcut viscosity , the midcut viscosity and the lowcut viscosity of whole blood and plasma viscosity all were comparable without a significant difference between the two groups(p>0.05). After the treatment, all of them decreased.All of the treatment group were significantly lower than those of the control group with a significant difference between the two groups(p <0.05).Conclusions:Starting from the etiology and pathogenesis of CSA——theobstruction of qi and blood and the disnourishment of the orifices, this study adopted the treatment by electroacupuncture combined with Danggui injection into points.The treatment obviously increases the symptom and function scoring of the patients with CSA enhances the clinical efficacy, and improves the mean velocities(Vm) of vertebral artery (VA) and basilar artery ( BA ), the tallcut viscosity, the midcut viscosity and the lowcut viscosity of whole blood and plasma viscosity. The treatment group is superior to the control group.Under the acupoint and the drug,the treatment can dredge the meridians,nourish the blood and invigorate the blood to make the qi and blood enough, the meridians unobstructed, the orifices nourished,the pain alleviated and the vertigo relieved. The mechanism of the treatment is probably that it can recover the balance of various physiological functions of cervical vertebrae by improving the blood supply of vertebro-basilar artery and decreasing the blood viscosity.These results demonstrate that it is scientific and feasible that CSA is treated of by electroacupuncture combined with Danggui injection into points.The treatment deserves to be popularized and applied in clinical practice.
