Objective To explore blood vessel anatomy, etiological and risk factors, clinical manifestations, imageology findings, diagnosis, treatment as well as prognosis on patients with top of basilar syndrome (TOBS) and thus to improve our knowledge about it. Methods By reviewing pertinent literatures, symptoms and physical signs of 21 cases with TOBS in our hosipital were retrospectively analysed. Results TOBS had special blood vessel anatomy features. Major etiological factors were arterial embolism and thrombosis, while risk factors were hyperlipemia, smoking, hypertension, high homocysteic acidemia, diabetes, heart diseases and so on. Main clinical manifestations comprised disorders of consciousness, oculomotor abnormalities, as well as pupillary abnormalities. Cerebral CT and MRI showed there were simultaneously more than two focus of infections in the midbrain, thalamencephalon, superior cerebellum and medial surface of temporal lobe, as well as occipital lobe. Therapeutic principle was basically the same as that of ischemic cerebrovascular diseases. However, it had poor prognosis and high case fatality and mutilation. Conclusions We should have adequate knowledge about the top of the basilar syndrome. Early diagnosis and treatment, as well as on guard against the appearance of many complications could improve its prognosis on patients with TOBS.
